经动脉灌注碳酸氢钠和维拉帕米提高抗肿瘤药物疗效的初步观察

【摘要】 目的 经动脉向恶性肿瘤组织灌注碳酸氢钠和维拉帕米 ,调节恶性肿瘤细胞内外H+ 、Ca2 + 离子分布 ,观察对IL 2、IFN α 2b和ADM、MMC等介入治疗的增效作用。方法 分层随机分为单纯动脉介入化疗药物组 (A组 ) ;碳酸氢钠联合化疗药物动脉介入组 (B组 ) ;碳酸氢钠联合IL 2、IFN α2b、化疗药物介入组 (C组 ) ;碳酸氢钠、维拉帕米联合IL 2、IFN α2b介入组 (D组 ) 4个组 ,A、B、C、D4个组介入 2个疗程后判断疗效。Seldinger技术穿刺经股动脉进入肿瘤动脉血管 ,药物按一定比例稀释后灌注。结果 B组部分缓解率明显高于A组 ,D组部分缓解率均高于其它 3个组。结论 向恶性肿瘤组织灌注碳酸氢钠、维拉帕米可提高IL 2、IFN α2b、ADM、MMC等疗效

【Abstract】 Objective To observe the increased efficacy of IL 2,ADM and MMC,by infusing NaHCO 3 and verapamil into malignant tumors and modifying distribution of H +?Ca 2+ in tumor cells.Methods Patients were divided into four groups randomly.Group A was infused with anti cancer agents alone;group B was infused with NaHCO 3 and anti cancer agents;group C with NaHCO 3,IL 2 and anti cancer agents;group D with NaHCO 3,verapamil,IL 2 and anti cancer agents.The efficacy was evaluated after two courses of arterial interventional chemotherapy in group A and B.And in group C and D the efficacy was evaluated after one course of treatment.By Seldinger technique,drugs were infused at proper concentrations in the following sequence:1.5% SB 80~100ml;2.NS 30ml;3.IL 2 40~60×10 4μ,4.ADM50~60mg;5.DDP 100mg or MMC10mg or DPT 50mg;6.5 FU 1.0~1.5;7.verapamil 10mg;8.verapamil 5~10mg in half hour.Results The partial remission (PR) in group B is significantly higher than that in group A;PR in group D is higher than those in group A,B and C.Conclusion Arterial infusion of NaHCO 3 and verapamil into malignant tumors can increase the efficacy of ADM,MMC and TL 2.
