尿路结石在超声影像中的新特征——彩色及能量多普勒声影

【摘要】 目的 :介绍尿路结石在彩色和能量多普勒扫描中显现出的一种与灰阶声影相当的彩色声影。可用来发现和辨认隐匿于不同背景中的尿路结石 ,提高对结石诊断的敏感性和准确性。方法 :使用宽频凸阵探头 ,对尿路可疑部位 ,变换进行二维灰阶、彩色和能量多普勒扫描。结果 :一年余在 2 0 7例尿路结石中 ,有 15 1例 (72 .9% )在 CD扫描中 ,紧随结石的强回声之后可见红蓝斑驳的彩色条带。同时也会在 PD扫描中 ,改呈饱和的桔红色条带。这两种彩色条带 ,比灰阶中的结石声影更具特征性 ,与之相对应 ,我们称其为彩色声影。结论 :认知彩色声影 ,特别有利于隐匿在尿路或混杂在窦区强回声中结石的显现。对尿路结石的检出和定位很有实用价值

【Abstract】 Objective:To introduce a new color sonographic feature of urinary stones for rapid and corrcet diagnosis.Methods:One hundred and fifty one (151/207,72 9%)patients (repeated cases included)were examined by alternative scanning with gray scale, color and power Doppler. All sonographic examinations were performed using wide frequency curvilinear phased array probe(2-5MHz), ATL 3500.Results:There were 81%(74/91) cases with renal stones, 67.4%(29/43) cases in upper ureter and 65.7%(48/73) cases in the lower ureter. All of these calculi possessed the color and power Doppler twinkling artifacts following strong echo of the calculi. We call these artifacts as color shadow because the features of artifacts which were are the same as the shadow on gray ultrasonography.had tail enough long as shadows. The artifacts are twinkling with red and blue speckle on color Doppler,and with an orange bright stripe on power Doppler. The images were then analyzed for difference in clinic and statistics. The color shadow were not related to the positions of uriary stones, to the focal zone, to the ultrasonic power,and to the Doppler velocity neither .The shadows would be related to the qualities of the stones.The appear dnce of the color shadow depended on the scan angle.Conclusions:Color Doppler and power Doppler twinkling artifacts from urinary stones were called as color shadow following strong echo of calculus and they may be considered as an additional sonograhpic feature of urinary stones. The visualization of these color shadow are very helpful in determining the presence and locations of urinary stones.
