【摘要】 目的 :探讨彩超在动脉导管未闭 (PDA)封堵术病例选择和疗效观察中的价值。方法 :封堵术前彩超测量 PDA的内径及长度、肺动脉内径、左房室大小等 ,观察 PDA位置及形状 ,封堵术后彩超探测封堵器位置 ,有无残余分流 ,肺动脉和左房室缩小情况。结果 :彩超检测 8例 PDA封堵术患者 ,二维测量导管平均长度 9.4 mm,平均内径 6 .5 mm,左向右分流平均速度 4 .3m/ s。术后 2～ 5天复查 ,肺动脉平均内径从 2 9mm缩小到 2 4 mm ,左室舒张末期平均内径从 6 0 mm缩小到 4 9mm,左房平均内径从 34mm缩小到 30 mm,3例封堵术后出现少量残余分流 ,其中 1例 2个月后复查残余分流消失。结论 :彩超检查有利于术前筛选 PDA封堵术患者 ,选择封堵器大小 ,术后检测有无残余分流 ,对心脏血流动力学评价以及疗效观察起到重要作用更多还原

【Abstract】 Objective:To probe the value of echocardiography in selecting patients with patient arterial duct who can be occluded and in observing curative effect.Methods:Measure the size of PDA?the diameter of pulmonary artery?the diameter of left atrium and ventricle before occlusion.Observe the position of the occlusion device and determine whether there is residual shunt.Estimate the changing of the pulmonary artery and left atrium and ventricle.Results:8 patients of patent arterial duct were assessed by echocardiography before and after occlusion.Mean echo diameter of PDA was 6 5mm and mean length of PDA was 9 4mm.The mean velocity of the shunt was 4 3m/s.The mean diameter of pulmonary artery?left atrium and ventricle were 29mm?34mm?60mm respectively.Echocardiograms were obtained again in 2～5 days after operation.The mean diameter of pulmonary artery?left atrium and ventricle were changed to 24mm?30mm?49mm respectively.Statistical analysis showed that they have been decreased significantly in all patients( P <0 01).A small shunt was detected in 3 cases,but it disappeared after 2 months.Conclusions:Echocardiography can be used for guidance to select occlusion device of PDA and detect whether there is residual shunt after operation.It is useful to evaluate hemodynamical change and curative effect.更多还原
