彩色多普勒能量图在浅表淋巴结病变诊断中的应用
Clinical Application of Color Doppler Power Imaging in Superficial Lymphadenopathy
【摘要】 目的 :比较彩色多普勒能量图与彩色多普勒血流显像在观察浅表淋巴结内血流状态的作用 ,探讨彩色多普勒能量图在淋巴结病变良恶性鉴别中的价值。方法 :应用彩色多普勒血流显像和彩色多普勒能量图观察 113个(良性 2 8个 ,恶性 85个 )浅表淋巴结内的彩色血流形态和分布。所有病例均经组织学证实 ,检查前均未进行过临床治疗。结果 :113个淋巴结内的彩色多普勒超声表现可分为 5种类型 ,即淋巴结门型、树枝状血流型、周边血流为主型、网状血流型和无血流型。与 CDFI相比 ,CDPI显示的血流数目更多 ,血管连续性更好。淋巴结门型和无血流型多见于良性淋巴结病变 ,恶性淋巴结病变主要表现为树枝状血流型、周边血流为主型和网状血流型。结论 :对于观察浅表淋巴结内血流状态 ,CDPI优于彩色多普勒血流显像。但是 ,CDPI对于提高超声对淋巴结病变诊断的准确性作用有限。
【Abstract】 Objective:To compare the abilities of color Doppler power imaging(CDPI)and color Doppler flow imaging(CDFI)in finding out the vascularity of superficial lymph nodes and to evaluate the usefulness of color Doppler power imaging in differentiating benign from malignant lymphadenopathy.Methods:CDFI and CDPI were used to evaluate 113 superficial lymph nodes in 113 untreated patients.Histological examination showed 8 lymphadenitis,20 reactively enlarged lymph nodes,70 malignant nodes containing metastases and 15 with lymphoma.Results:Five vascular patterns were identified:type Ⅰ(a single vascular pole in the hilum),type Ⅱ(hypertrophic single vascular pole with two or more branched arteries),type Ⅲ(peripheral vascularity),type Ⅳ(netty vascularity)and type Ⅴ(absence of perfusion).The type Ⅰ and Ⅴ were seen more often in benign nodes;the type Ⅱ?Ⅲ and Ⅳ were mainly related to malignant nodes.Conclusions:CDPI is superior to CDFI in the assessment of nodal vascularity,but not more effective than CDFI in differentiating benign from malignant lymphadenopathy.
