【摘要】 目的　探讨各种疑难、复杂的白内障超声乳化术中难点及手术技巧。方法　应用超声乳化前深埋手法劈核、有效超声乳化、非创伤性小瞳孔牵拉扩张、小瞳孔原位超乳碎核等法对棕黑硬核、成熟期、过熟期、悬韧带松弛、部分断裂、晶体半脱位、小瞳孔、超高度近视、超短眼轴、小眼球、浅前房等复杂性白内障 62 8例 75 6眼顺利完成超声乳化手术。结果　术后 1月矫正视力≥ 0 5以上共 5 5 1眼 (72 9% ) ,角膜水肿 2 6眼 (3 4% ) ,后囊破裂 3 7眼 (4 9% ) ;2 18眼小瞳孔眼中 2 10眼术后能保持生理性圆瞳孔 (96 3 % ) ;2 3眼晶体悬韧带松弛、部分断裂或半脱位 ,其中 ,2 1眼顺利植入人工晶体 (91 3 % )。视网膜脱离 1眼(0 13 % )。结论　复杂性白内障并非手术禁忌症 ,应用合理的手术技巧可望获得良好效果更多还原

【Abstract】 Objective To analyse and investigate the difficulty,settlement and surgical technology in various complicated cataract.Methods By manual chop with embedding,effective phacoemulsification,non traumatic dilation of small pupil,in situ phacoemulsification in small pupil and stuffing,successful phacoemulsification in 628 cases (756 eyes) with various complicated cataract,included:brownish black nuclear,hypermature cataract,relazed susensory ligament of lens or partial fragemention of susensory ligament of lens,semi subluxation of the lens,small pupils,ultrahigh axial myopia,ulrashort ocular axis,microphthalmus,shallow anterior chamber and ect.Results The proportions of corrected visual acuity of 0 5 or better were 551 eyes (72 9%),reversible moderate corneal edema 26 eyes (3 4%),tear of the posterior capsule 37 eyes (4 9%).210 in 218 eyes (96 3%) with small pupils had physiological round pupils after operation.Intraocular lens implantation through 21 in 23 eyes (91 3%) with relaxation or partial fragementation susensory ligament of lens or semi subluxation of lens.1 eyes (0 13%) was retinal detachment.Conclusion The result suggest that the complicated cataract is not the contraindication of the phacoemulisification.Good effect can be obtained by corresponding and reasonble surgical technology.更多还原
