【摘要】 目的　探讨小梁切除术联合透明晶状体摘出治疗原发性闭角型青光眼的疗效和适应证。方法　小梁切除术联合同一切口的透明晶状体超声乳化及折叠人工晶状体植入治疗原发性闭角型青光眼 2 6眼 ,术后平均随访 16个月( 12～ 18个月 )。结果　眼压由术前 ( 2 9 82± 7 82 )mmHg降至术后 ( 13 94± 5 76 )mmHg ,其中 88 4 6 %的患眼不需加用抗青光眼药物即可将眼压控制在 2 1mmHg以下 ,5 0 %的患眼术后裸眼视力在 0 5以上。术前眼轴均值为 ( 2 2 6 6± 0 96 )mm ,晶状体厚 ( 4 86± 0 2 2 )mm ,前房深度 ( 2 35± 0 2 8)mm ,术后前房深度 ( 4 10± 0 37)mm。术后浅前房的发生率明显低于单纯接受小梁切除术的对照组。结论　对明显有晶状体增厚、浅前房以及短眼轴特征的原发性闭角型青光眼 ,三联手术较单纯小梁切除术有较好的疗效。更多还原

【Abstract】 ObjectiveTo study the indications and efficacy of trabeculectomy combined with clear lens extraction for primary angle-closure glaucoma.Methods26 eyes with clear lens were treated with trabeculectomy combined with phacoemulsification and foldable intraocular lens implantation through one incision.Follow-up period was 12～18 months.ResultsThe average intraocular pressure was reduced from (29.82±7.82)mmHg of preoperation to (13.94±5.76)mmHg of postoperation.Intraocular pressure was controlled under 21 mmHg in 88.46% of surgeried eyes.The visual acuity of 50% eyes was 0.5 or better.The preoperative axial length was (22.66±0.96)mm,lens thickness was (4.86±0.22)mm,anterior chamber depth was (2.35±0.28)mm;the postoperative anterior chamber depth was (4.10±0.37)mm.The incidence of shallow anterior chamber was lower than that of control group which received only trabeculectomy.ConclusionsTriple procedure provides good results in primary angle-closure glaucoma with thick lens,shallow anterior chamber and short axial length.更多还原
