L-精氨酸对随意皮瓣存活的影响
Effects of L-arginine on survival of random pattern skin flap
【摘要】 目的 探讨L -精氨酸 (L -arg)对随意皮瓣存活的影响及其作用机制。方法 选择雄性SD大鼠 34只 ,随机分组 :对照组 12只 ,实验组 2 2只。在鼠背部设计制备单蒂型随意皮瓣 (含肉膜层 ) ,长宽为 70mm× 15mm ,原位回植。实验组术后立即腹腔注射L -arg(1g/kg)加 2ml/kg生理盐水 ,对照组术后立即腹腔内注射 2ml/kg生理盐水 ;术后 2 4h取皮瓣组织检测一氧化氮 (NO)、超氧化物歧化酶 (SOD)、丙二醛 (MDA)的含量 ,术后 7天测量两组皮瓣存活长度。结果 实验组术后 2 4hNO及SOD含量均高于对照组 (P <0 .0 1) ,而MDA含量低于对照组 (P <0 .0 1) ;实验组皮瓣存活长度也明显高于对照组 (P <0 .0 1)。结论 L -arg对随意皮瓣存活有促进作用。其作用机制可能是增加外源性L -arg的量 ,促进了机体组织中NO的生成 ,进而使组织中SOD增多 ,并降低了机体组织中的MDA ,改善皮瓣血供 ,减轻组织缺血 ,促进了创面愈合 ,增加了皮瓣存活率。
【Abstract】 Objective To investigate the influence of L-arginine (L-arg) on random pattern skin flap and its mecnanism. Methods Thirty four male SD rats weighing 250 to 400 g, were randomly divided into a control group (n=12) and an experiment group (n=22). Dorsal skin flaps measuring 15 mm×70 mm were raised, the raised flaps were sutured back in the original place. Immediately after the operation, L-arg (1 g/kg) in 2 ml/kg saline was administered intraperitoneally to the experiment group, and an equal volume of saline to the control group. The NO, SOD and MDA contents in the flap were examined 24 h after operation, and the length of survival flap in the 2 groups were examined 7 d after operation.?Results The NO and SOD contents were significantly higher and the MDA was significantly lower 24 h after operation in the experiment group than in the control group (P<0.01). The length of survival flap in the L-arg-treated group was significantly increased (P< 0.01), compared with the control.?Conclusion Exogenous arginine may promote the survival of random skin flap. The higher survival length of the flap might owe to the mechanism that the supplement of exogenous arginine helps the flap tissue to produce more NO, and more SOD as well, while reducing the production of MDA in the flap. All these changes improve the local microcirculation, promote wound healing and increase the survival length of the flap in the experiment group.
