补镁对糖耐量异常、高胰岛素及高胰岛素原血症的影响——附20例报告
Impact of magnesium supplementation on glucose tolerance,hyperinsulinemia and hyperproinsu linemia:report of 20cases.
【摘要】 目的:探讨补镁对糖耐量异常(IGT)、轻型2型糖尿病的高胰岛素血症及高胰岛素原血症的影响。方法:选择空腹血糖5mmol/L~7mmol/L,谷氨酸抗体阴性的糖耐量异常和糖尿病患者36例,并分成治疗组20例,予25%硫酸镁10mL加入生理氯化钠250mL中静脉滴注,对照组16例给予生理氯化钠溶液,1次/日,用药1周后行口服葡萄糖耐量试验(OGTT),比较两组血糖、胰岛素释放曲线下面积及空腹胰岛素原水平的变化。结果:治疗组和对照组的血糖曲线下面积分别为(28±5)mmol·h·L-1和(32±4)mmol·h·L-1,P<0.01;胰岛素曲线下面积分别为(297±95)u·h·L-1和(584±242)u·h·L-1,P<0.01;胰岛素原水平为(5.5±3.0)pmol/L和(12.8±5.6)pmol/L,P<0.01;空腹胰岛素原/胰岛素比值也由对照组的33%降为治疗组的24%(P<0.01)。结论:补镁能明显降低IGT和轻型2型糖尿病患者升高的OGTT血糖和胰岛素曲线下面积、降低空腹胰岛素原水平,改善胰岛素抵抗,恢复受损的胰岛B细胞分泌功能。
【Abstract】 Objective:To study the impact of magnesium supplementation on hyperinsulinemia an d hyperproinsulinemia in patients with abnormal glucose tole rance and mild diabetes.Methods :Patients who had abnormal glucose to lerance and mild diabetes with fasting blood sugar lever of 5mmol /L ~7mmol /L and negative anti-glu tamate antibody were divided into tw o groups:20in treatment group receiving daily inf usion of 10mL of 25%magnesium sulphate in 250mL of normal saline and 16in c ontrol group re-ceiving only normal saline infusion.OGTT was performed after one week of treatment.Blood sugar level,area under insulin releasing curve and fasting proinsulin level were compared between the two groups.Results:The area under blood sugar curve in tr eatment and control group was(28±5)mmol·h·L -1 and(32±4)mmol·h·L -1 ,respectively(P<0.01);The area under insulin curve was(297±95)u·h·L -1 and(584±242)u·h·L -1 respectively,P<0.01;The proinsulin level was(5.5±3.0)pmol /L and(12.8±5.6)pmol /L respectively,P<0.01;The fasting proinsulin /insulin ratio in control group was reduced from 33%to 24%in the treatment group(P<0.01).Conclusion:Magnesium supplementation can sign ificantly reduce the area under OGTT blood sugar curve and insulin curve,lower fasting proinsulin level,improve insulin resistance and restore secretory function of islet B cells in IGT and mild type 2diabetes mellit us.[
