【摘要】 目的 :探讨超声内镜 (EU S)对常规内镜检查怀疑为恶性胃溃疡的诊断价值。方法 :应用 Pentax-36 ux型 EUS对常规内镜检查怀疑为恶性胃溃疡的患者进行检查 ,EU S下的良性溃疡行正规内科治疗后复查 ,恶性溃疡均经外科手术证实。结果 :经 Pentax- 36 ux型 EUS诊断的患者 18例 ,良性胃溃疡 6例 (33.3% ) ,经正规内科治疗后二月胃镜复查均痊愈 ,准确率 10 0 % ;恶性胃溃疡 12例 (6 6 .7% ) ,通过与术后病检进行对照 ,准确率为 91.7% (11/ 12 ) ,假阳性率为 8.3% (1/ 12 ) ,同时把 EU S下恶性胃溃疡的 TNM分期与术后对比。结论 :EU S能很好地鉴别良、恶性胃溃疡 ,由于 EU S下恶性胃溃疡的 TNM分期有较高的准确性 ,对外科手术方式选择有很大帮助 ,应把 EU S作为恶性胃溃疡手术前一项重要的检测手段更多还原

【Abstract】 Objective: To study the diagnosis assessment by Ultrasonic endoscope (EUS) in malignant gastric uncler suspected by routine endoscopy. Methods: Examination using Pentax 36ux EUS was carried out in the patients with malignant gastric uncler suspected by routine endoscopy. Benign gastric uncler diagnosed by EUS repeated examination using endoscopy after normal medical treatment; Surgery was carried in all patient with malignant gastric unclers diagnosed by EUS. Result: Of the 18 patients diagnosed by Pentax-36ux, 6 patients(33.3%) were benign gastric uncler, they were all recovery in two months after normal medical treatment with repeating endoscopy, the accuracy by EUS was 100%; 12 patients(66.7%) were malignant gastric uncler, compared with operation, the accuracy was 91.7%(11/12). The false positive rate was 8.3%(1/12). Meanwhile, malignant gastric uncler TNM stages by EUS was compared with operation. Conclusion: EUS can indentify benign or malignant gastric uncler very well. Because of the high accuracy of malignant gastric uncler TNM stages , EUS was very advantageous for chosing surgery mode. Accordingly, EUS should be served as a responsible techniques before operating of malignant gastric uncler.更多还原
