窒息新生儿多种内分泌激素的变化及其临床意义

【摘要】 为研究新生儿窒息缺氧时瘦素、胰岛素、生长激素、甲状腺素、糖皮质激素及体重的变化并探讨其临床意义 ,应用放射免疫法测定 13例中度、10例重度窒息患儿及 10例无窒息对照组新生儿静脉血血清上述 5种内分泌激素水平。发现中、重度窒息患儿生后 5 d体重增长下降 (P<0 .0 5 ) ;出生后 2 4~ 4 8h血清生长激素、甲状腺素水平明显低于对照组 (P<0 .0 5 ) ,而瘦素、皮质醇、胰岛素明显高于对照组 (P<0 .0 5 ) ;出生后第 5 d中度窒息组上述 5种内分泌激素水平接近正常 ,而重度窒息组部分激素水平仍明显异常。提示 :窒息缺氧引起患儿多种内分泌变化 ,不利于生长发育 ,窒息程度重者影响更大

【Abstract】 To study the changes of leptin, insulin, growth hormone, thyroxine, cortisteoid and body weight in neonatal asphyxia, 13 newborns with moderate asphyxia and 10 newborns with severe asphyxia were studied, and 10 nonasphyxia newborns were set as control group. Plasma leptin, insulin, growth hormone, thyroxine and corticosteoid were determined by radioimmunoassay. Asphyxia resulted in a significant decrease in body weight ( P <0 05), plasma growth hormone ( P <0 01), thyroxine ( P <0 05), and an increase in leptin ( P <0 05), corticosteoid ( P <0 01), insulin ( P <0 05). All hormonal variables of newborns with moderate asphyxia had normalized at the fifth day in life, but partial hormonal variables of newborns with severe asphyxia were abnormal. It indicated that the endocrine changes of the asphyxia newborns go against the growth and development.
