【摘要】 目的探讨强直性脊柱炎与致密性骨炎的临床及影像学鉴别诊断。方法对38例强直性脊柱炎,21例致密性骨炎临床表现与影像检查进行回顾性分析对比。结果强直性脊柱炎与致密性骨炎均好发于骶髂关节附近,二者最常见的临床症状均为下腰部疼痛,而强直性脊柱炎除有下腰部疼痛外,往往伴有僵硬感。影像方面,强直性脊柱炎常损害骶髂关节,造成骶髂关节的狭窄及强直,伴新骨生成及下腰部韧带骨化。而致密性骨炎病变多只限于髂骨部,并不侵犯关节面。结论CT检查对强直性脊柱炎与致密性骨炎的诊断及鉴别诊断价值很大。更多还原

【Abstract】 Objective To investigate the differential dia gnosis between ankylosing spondyli tis(AS)and condensing ostitis(CO)and to analyze their clinical manifesta tions and CT features.Methods Clinical manifestations and CT find ings of 38patients with AS and 21patients with CO,who were confirmed by clinical and imaging data were analyzed retrospectively and compared each other.Results The most common symptom in both of AS and CO were lower lumbar pain whereas lumbar rigidity sense was only found in patien ts with AS.CT re-vealed that the lesions of AS frequen tly involved into sacroiliac joints and led up to their narrowing and stif fness or complicated with new bone formation and ligamental ossif ication whereas the lesions of CO were only limited in the ilia without any invasion of joint sur-faces.Conclusion CT plays important role in the differential diagnosis between AS and CO.[更多还原
