桥小脑角区肿瘤及肿瘤样病变的CT诊断

【摘要】 目的探讨CT对桥小脑角区肿瘤及肿瘤样病变的诊断价值。方法回顾性分析经手术病理证实的桥脑小脑角区肿瘤及肿瘤样病变的CT特征。其中听神经瘤9例、胆脂瘤4例、脑膜瘤3例、耳源性脑脓肿5例、三叉神经瘤1例。结果各种肿瘤的CT特征,随其种类而不同,如听神经瘤均位于内听道开口处,呈等或低密度团块,轻度均匀或环状强化。胆脂瘤呈球形或扁平形,球形位于硬膜外,密度不均,壁有钙化。扁平形位于蛛网膜下腔,边缘清楚,均匀低密度。脑膜瘤均具有典型脑膜瘤特征,明显强化。耳源性脑脓肿周边有明显水肿、囊壁清晰,囊内呈均匀低密度。三叉神经瘤跨中后颅窝伴岩锥尖骨质破坏。结论桥脑小脑角区肿瘤各具特点,能提供明确诊断。

【Abstract】 Objective To evaluate the use of CT in diagnosing the cerebral tumor and tumor-like lesions arising from cerebellopontine angle areas(CPAA).Methods CT features of cerebral tumor and tumor-like lesion arising from CPAA in 22 patients with surgically and pathologically proven auditory nerve tumor(n=9),cholesteatoma(n=4),meningioma(n=3),ear-genetic brain abscess (n=5),and trigeminal nerve tumor(n=1)were retrospectively analyzed.Results CT features of above mentioned tumor and tumor-like lesions are different each other with their kinds.For example,auditory nerve tumor lying at the opening of internal auditory canal and appearing as isodense or hypodense mass with light homogeneous or circular enhancement,Cholesteatoma appearing as spheric or flat shape with nonhomogeneous or homogeneous hypodense,meningioma having characteristic findings and obvious inhancement,ear-genetic abscess to be surrounded by obvious edema,having clear cystic wall and intracystic homogeneous hypodense,trigeminal nerve tumor transcending middle-posterior cranial fossa with bony destruction.Conclusion If only remembering CT features of above montioned tumor and tumor-like lesions,the diagnosis and differential diagnosis are not difficult.
