【摘要】 目的　探讨颈动脉窦按压试验阳性原因不明的晕厥患者的病因。方法　对近年以晕厥原因待查入院的 2 8例患者行颈动脉窦按压试验 ,4例患者颈动脉窦按压试验阳性。对这 4例患者的病例特点、病因进行了探讨和随访。结果　 4例患者在按压左或右侧颈动脉窦时心电图显示心动过缓、窦性停搏或II度房室传导阻滞。最长的RR间期从 2 4s至 11 0s,血压下降 4 0～ 6 0mmHg(1mmHg =0 133kPa)。进一步临床检查并长期随访显示最终病因为颈动脉窦过敏综合征 1例 ,颈动脉窦过敏合并病态窦房结综合征 1例 ,颈动脉体瘤 1例 ,脑血管烟雾病(moyamoya综合征 ) 1例。结论　对于颈动脉窦按压试验阳性原因未明的晕厥患者 ,不能立即将其病因归咎于颈动脉窦过敏综合征 ,而应进一步探讨其病因 ,尤其是年轻患者更多还原

【Abstract】 Objective\ To investigate the underyling cause of unexplained syncope with carotid sinus massage positive. Method\ 28patients with unexplained syncope underwent carotid sinus massage test. At last 4 patients were diagnosed as carotid sinus massage positive.We investigated the features and cause of the 4 cases.Results\ When stimulating the right or left carotid sinus, ECG of the four patients revealed sinus bradycardia, sinus arrest or type II atrial ventricular block. The longest RR interval was from 2 4 seconds to 11 0 seconds with systolic blood pressure dropping about 40～60mmHg. After further investigation and long term follow-up, the final causes of syncope were carotid hypersensitive syndrome for one case, carotid hypersensitive with sick sinus syndrome for one case, carotid body tumor for one case and moyamoya syndrome for one case. Conclusion\ When a patient of unexplained syncope had a positive carotid sinus massage test, we can’t conclude immediately that the cause of syncope is carotid hypersensitivity, we should look for the underlying cause of syncope especially in a young patient.更多还原
