螺旋CT及3D重建在胫骨平台骨折诊疗中的价值
Evaluation of Spiral CT and 3D Reconstruction on Diagnosis and Treatment of Tibial Plateau Fractures
【摘要】 目的 评价螺旋CT及 3D重建在胫骨平台骨折诊疗中的价值。方法 自 1998-0 6~ 2 0 0 0 -12我们对 2 5例胫骨平台骨折进行普通X线摄片、螺旋CT及 3D重建检查 ,按Schatzker分类法 ,对X线摄片及 3D重建图像分型 ,且在胫骨平台俯视图上将平台骨折分为前外、后外、前内、后内象限骨折。结果 2 5例平台骨折 3D重建图像的胫骨平台分型和关节镜所见完全一致。在 3D重建图像的指导下 ,2 5例骨折进行手术 (其中 2 3例在关节镜下微创治疗 ) ,无 1例伤口感染 ,疗效肯定。结论 螺旋CT及 3D重建在助于胫骨平台正确分型 ,并补充了在三维空间上新的骨折分型 ,对手术入路 ,复位和内固定的选择有指导意义
【Abstract】 Objective To evaluate the value of spiral CT and 3D reconstruction in diagnosis and treatment of tibial plateau fractures.Methods 25 cases with tibial plateau fractures were examined with plain radiograhs,spiral CT and 3D reconstruction during July,1998 to December,2000.The tibial plateau fractures were classified according to Schatzker′s classification on the X-rays and 3D reconstruction images.On the superior view the tibial plateau fractures were divided into anterior lateral,posterior lateral,anterior medial and posterior medial fractures.Results 3D reconstruction images of 25 plateau fractures were the same as what arthroendoscopy were.25 fractures(including 23 arthroscopic management fractures)were operated under 3D reconstruction guide.No one case was infected.The treatment was valid.Conclusion Spiral CT and 3D reconstruction can offer more accurate classification and complement to new spatial classification of fractures.Also can provide better planning and management for the tibial plateau fractures.
