调Q Alexandrite激光治疗太田痣的合理间隔时间
Study on the Treatment Interval in Q-switched Alexandrite Laser Therapy of Nevus of Ota
【摘要】 目的 探讨调QAlexandrite激光对太田痣的合理治疗间隔时间。方法 2 67例接受 3次治疗的患者按治疗间隔分为 4组 ,并进行疗效评判 ;对 187例患者就术后色泽消退停止的时间进行调查 ;5例患者接受了多例次活检 ,对 7例次术后采集的标本进行光镜和电镜观察。结果 所有患者均有不同程度色素消退 ,无一例产生瘢痕。治疗间隔 5~ 6月者疗效明显优于 3~ 4月 ,而与 7~ 8月及 9月以上者相比疗效无显著差异。 80 .2 1%患者至术后 7月已观察不到明显的色素消退。光镜下 ,术后 4月左右噬黑素细胞明显增加 ;此时在电镜下可见黑素小体与细胞残骸仍散在于胶原纤维之间。结论 大多数太田痣患者的合理治疗间隔为 5~ 6月
【Abstract】 Purpose To find out the appropriate treatment interval in Q switched Alexandrite laser therapy of Nevus of Ota. Methods 267 patients undergoing 3 treatment sessions with Q switched Alexandrite laser were divided into 4 groups according to treatment interval, and clinical response was compared between these 4 groups. 187 patients were inquired of the process of pigment fading. Moreover, 5 patients received multiple biopsies, and altogether 7 specimens after irradiation were available for light and transmission electron microscopy. Results Cosmetic improvement without scar formation was observed in all the patients. The clinical response of 5-6 month treatment interval group was significantly better than that of 3-4 month interval group, while showed no significant diffe rence as compared with both 7-8 month and ≥9 month interval group. 80.21% of investigated patients stated that remarkable pigment fading could no longer be observed 7 months after irradiation. It was disco vered under the light microscope that melanophage increased significantly in number around 4 months after irradiation. Meanwhile, transmission electron microscopy showed that degenerated melanosomes as well as cell debris were still scattered among collagen bundles. Conclusions We recommend that appropriate treatment interval be 5-6 months for most patients.
