高频震荡通气与常规通气对ARDS肺损伤的影响
The Influence of High-Frequency Oscillation Ventilation and Conventional Mandatory Ventilation on Lung Injury Development in ARDS Animal
【摘要】 目的观察高频震荡通气与常规通气对急性呼吸窘迫综合征 (ARDS)动物肺损伤发展的影响。 方法通过生理盐水肺灌洗 ,建立ARDS动物模型 ,分别应用高频震荡通气 (HFOV)和常规通气 (CMV) ,6h后再行肺泡灌洗 ,灌洗液细胞分类 ;处死动物 ,肺组织分别作干湿重测定、光镜和电镜检查。 结果HFOV和CMV两组间动脉血气分析 (pH、PaO2 、PaCO2 )无显著性差异。HFOV和CMV治疗对肺泡灌洗液中嗜中性粒细胞数目的变化无显著性影响 ,对反应局部肺水肿状况的湿 干重比值亦无显著性影响。但与CMV相比 ,HFOV治疗不仅可显著减少肺损伤的范围 ,而且可降低光镜和电镜的肺损伤评分。 结论在获得同样的动脉血气分析结果条件下 ,与CMV相比 ,HFOV显著减少ARDS动物肺损伤的发展
【Abstract】 Objective To explore the influence of high-frequency oscillation ventilation (HFOV) and conventional mandatony ventilation (CMV) on lung injury development in ARDS animals and to define the role of HFOV in the treatment of ARDS. Methods ADRS animal models were prepared by lung lavage with saline. After intubation, the animals were treated with HFOV or CMV (n=6). PaCO 2 was maintained between 35~45mmHg by adjusting the parameters of ventilator. The ventilation period was 6h. Lung lavage was given after ventilation, and cytological examination of the lavage fluid was performed. Then the animals were killed and lung tissues were removed for wet/dry weight ratio, light and electron microscopic examination.Results The difference of artery blood gas analyses (pH、PaO 2、PaCO 2) between HFOV and CMV was insignificant. There was no significant difference between HFOV and CMV in the cytological examination of lavage fluid and wet/dry weight measurement. But compared with CMV, HFOV not only reduced the area of lung injury, but also reduced the grade of lung injury in light and electron microscope examination. Conclusion When the same artery blood gas analyses. HFOV can reduce lung injury development significantly. As a lung protection strategy, HFOV can be used in the treatment of ARDS.
