胶体金免疫层析法在脑囊虫病诊断中的应用

【摘要】 目的 探讨胶体金免疫层析法在脑囊虫病诊断中的价值。方法 应用胶体金免疫层析法检测脑囊虫病患者脑脊液 (CSF)及血清中的循环抗原 (CA) ,并与ELISA比较。结果 检测脑囊虫病人CSF和血清各 48份 ,阳性检出率分别为 81.2 5 %和 70 .83%。检测健康人CSF和血清各 2 6份 ,阴性符合率分别为 92 .31%和 90 .0 0 %。检测 2 3份非囊虫病 (脑肿瘤、脑脓肿、脑结核、脑外伤 )患者CSF和血清的交叉率分别为 8.70 %和 11.32 %。检测包虫病患者、血吸虫病患者和华支睾吸虫患者的血清各 10份 ,交叉率分别为 2 0 .0 0 %、10 .0 0 %和 0。敏感性和特异性与ELISA相近。结论 胶体金免疫层析法用于脑囊虫病诊断具有较高的敏感性和特异性 ,操作简便、快速 ,无需特殊仪器设备 ,有较好的应用价值。

【Abstract】 Objective To evaluate the application value of the immunochromatographic test for the diagnosis of neurocysticercosis. Methods Assess the sensitivity and specificity of the immunochromatographic test which is based on detection of circulating antigen(CA) in cerebrospinal fluid (CSF) and sera of patients with neurocysticercosis and compared it with ELISA. Result The positive rate of CA in CSF of 48 patients with neurocysticercosis was 81.25%. 53 of 72 sera from patients with neurocysticercosis’were positive. The positive rate was 73.61%.The negative coincidence rate of 26 CSF and 40 sera from normal population was 92.31% and 87.50% respectively. The cross-reaction rate of 23 CSF and 23 sera from patients with other diseases was 8.70% and 11.32% respectively. The cross-reaction rate of 10 sera from hydatidosis patients and 10 sera from schistosomiasis patients was 20.00% and 10.00% respectively. All of 10 sera of clonorchiasis cases were negative. This test and ELISA had a similar sensitivity and specificity. Conclusion This assay was sensitive, specific and simple, it is suitable for diagnosis of neurocysticercosis.
