【摘要】 目的　探讨颈动脉加压注射尿激酶治疗缺血性脑梗死的疗效。方法　采用前瞻性多中心随机方案 ,将入选患者随机分为两组 ,颈动脉加压注射尿激酶组 (治疗组 ) 132例 ,常规治疗组 (对照组 ) 89例 ;疗效判定依据1986年中华医学会全国第 2次脑血管病学术会议修订的项目制定。结果　 (1)两组疗效比较 ,差异有非常显著性意义 (P <0 0 0 1) ;(2 )治疗开始的时间≤ 48小时与 >48小时者的疗效比较无论是治疗组还是对照组 ,差异均有显著性意义 (P <0 0 0 1) ;(3)对治疗组脑梗死急性期的疗效与恢复期及后遗症期相比差异有显著性意义 (P <0 0 0 5 )。结论　颈动脉加压注射尿激酶治疗缺血性脑梗死 ,提高了缺血区药物浓度 ,克服了全身用药所致的高费用低疗效 ,疗效明显优于常规方法 ,降低了致残率 ,值得临床推广使用。更多还原

【Abstract】 Objective To evaluate the therapeutic effect of carotid adding pressure injecting urokinase on cerebral ischemic infarction.Methods A prospective,multicenter,randomized clinical trial was conducted.Patients enrolled were randomly divided into two groups:the treatment group (carotid adding pressure injecting urokinase group) and the control group (routine treatment group).The standards of the effective determination was drawn up according to the criteria of the Chinese Medical Association,which was revised in the second conference of cerebral vascular disease.Results (1)The total effect rate and close care rate in treatment group were significantly higher than those in control group(P<0 005).(2)The total effect rate of the patients in treatment group (treatment time≤48h or >48h) was significantly higher than that of control group(P<0 05).(3)The effect rate of patients with cerebral infarction in acute period was significantly higher than that in convalescent period(P<0 005).Conclusion The method of carotid adding pressure injecting urokinase could be used to increase agent level in ischemia area and decrease the patients′ mutilation rate.更多还原
