梅尼埃病多因性初探

【摘要】 目的 :探查梅尼埃病发病的可能原因。方法 :对临床诊断符合梅尼埃病 2 4例在症状发作期分别检测其脑血管多普勒(TCD) ,血浆内皮素 (ET) ,血栓素B2 (TXB2 )与循环免疫复合物 (CIC)。另取正常对照组 2 4例作以上相同项目测试。结果 :梅尼埃病组 2 4例TCD峰值降低 18例 ,ET升高 16例 ,TXB2 升高 17例 ,CIC升高 6例 ;正常对照组 2 4例TCD峰值降低 7例 ,ET升高 4例 ,TXB2 升高 5例 ,无 1例CIC升高。两组比较差异显著 (P <0 .0 5 )。结论 :因血管因素导致的内耳供血、供氧障碍可能是梅尼埃病发病的重要原因。免疫反应也是梅尼埃病致病因素之一。梅尼埃病很可能是由多种致病因素引起的具有三联征的综合征。

【Abstract】 Objective: To explore the probable pathogenic cause of Meniere’s disease. Methods: Transcranial doppler(TCD), plasma endothelin(ET), thrombxaneB 2(TXB 2) and circular immune compound(CIC) were tested respectively in twenty- four cases whose clinical diagnosis were consistent with Meniere’s disease at its symtomatic onset, whereas same items were tested in twenty- four normal control group. Results: In Meniere’s group, TCD peak value reduced in eighteen cases, ET rised in sixteen, TXB 2 rised in seventeen and CIC rised in six cases, whereas In normal control group, TCD peak value reduced in seven cases, ET rised in four, TXB 2 rised in five and none case rised as to CIC. The difference is evident between two groups. Conclusion: The obstacle of blood supply and oxygen supply caused by vascular factors is probably the important cause of Meniere’s disease, also immune reaction is one of the pathogenic factor. Most probably, Meniere’s disease is really a syndrome which has three-connecting sign caused by various factors.
