【摘要】 目的　探讨对病毒性心肌炎 (VMC)的早期诊断的价值。方法　选择外周血检测柯萨奇病毒特异性抗体IgM (CBV -IgM )阳性的柯萨奇B组病毒性心肌炎患者 5 4例 ,依据心电图 (ECG)表现分三组 :A组 (ECG正常 ,n =10 ) ,B组 (ECG单项异常 ,n =37) ,C组 (ECG重叠异常 ,n =7) ,同期选择 32名健康志愿者为对照组 ,采用ELISA法定量测定血清cTnI、cTnT。结果　 5 4例VMC患者中 ,血清cTnI水平显著高于对照组(10 5± 7 6 3ng L 1vs 1 87± 0 74ng L-1,P <0 0 0 1) ;cTnI阳性率 (70 4 % )显著高于cTnT (5 7 4 % ) (P <0 0 1) ;B、C组分别与A组比较 ,血清cTnI、cTnT水平均显著升高 (P <0 0 1) ,C组与B组比较 ,仅血清cT nI水平升高有显著性意义 (P <0 0 1) ,cTnT水平升高无显著性差异 (P <0 0 5 )。结论　血清cTnI可作为VMC早期诊断的指标 ,且有助于VMC病情判断更多还原

【Abstract】 Objective To evaluate the clinical value of serum troponin I(cTnI)in acute viral myocarditis.Methods 54 patients with positive CBV-IgM were studied.By measures of electrocardiogram(ECG),The total cases were divided into three groups:group A with normal ECG ( n =10),group B with one species of abnormal ECG n =37),group C with two or above abnormal ECG ( n =7).The normal subjects was consisted of 32 healthy volunteers.Blood samples were obtained at admission.Results Serum concentration of cTnI was markedly higher in VMC patients than that in normal subjects(10 5±7 63 ng L -1 vs 1 87±0 74 ng L -1 , P <0 001) The total positive rates of cTnI and cTnT were 70 4%(38/54) and 57 4% respectively. The positive rates of cTnI was significantly higher than cTnT( P <0 01) When compared group C and B,serum cTnI levels in group C were singnificantly higher than those in group B( P <0 01),and no singanificant difference was found in cTnT( P >0 05). Conclusion Compared with cTnT,cTnI is much more sensitives and specified,which indicated that the cTnI should act as a valuable measurement in diagnosis of acute viral myocarditis.更多还原
