【摘要】 目的　探讨甲泼尼龙与环磷酰胺联合冲击治疗大疱性类天疱疮的临床疗效与不良反应。方法　甲泼尼龙连续用药 3d为 1个疗程 ,第 1,2 ,3d分别静滴 30 0mg ,2 0 0mg ,2 0 0mg ,必要时 1周后行下一疗程治疗 ;环磷酰胺 6 0 0mg ,静滴 ,每周 1次。结果　 8例病人中 1疗程控制 7例 ,2疗程控制 1例 ,2月内皮质类固醇激素 (泼尼松 )减量至 5 0mg/d以下 ;冲击治疗期间 ,白细胞总数、血压有不同程度增高 ,2例出现低血钾 ,糖尿病患者血糖变化明显。结论　本疗法具有起效快、病情控制迅速、有利于激素减量 ,是治疗大疱性类天疱疮的有效的治疗方法之一 ,但对有合并症患者应慎用更多还原

【Abstract】 Objective To study the combined impact treatment effect of the methylprednisolone and cyclophosphamide on the bullous pemphigoid and the side effects of the drugs Methods A course of methylprednisolone treatment consisted of 3 days, the 300 mg, 200 mg and 200 mg of the methylprednisolone was given via intravenous drip at the first, the second and the third day respectively, if it was necessary the second course of the treatment can be given one week after the first course of treatment The 600 mg cyclophosphamide was given via intravenous drip 1 time/week Results The 7 patient’s condition of the 8 patients was controlled after one course of treatment, one patient’s condition of 8 patients was controlled after two courses of treatment, the dose of glucocorticoid(prednisone)was decreased under 50 mg/d during 2 months; During the phase of the combined impact treatment, the total leucocytes and the blood pressure of the patients were raised at the different levels, two cases had the hypokalemia, the blood sugar of the diabetes mellitus patients was changed obviously Conclusion The combined impact treatment can effect rapidly, the patient’s condition can be controlled rapidly, the dose of hormone can be decreased easily, so this treatment was one of the effective treatment methods which were used to treat the bullous pemphigoid, but this treatment should be cautiously used to treat the patients with the complications更多还原
