【摘要】 目的 :了解小儿系统性红斑狼疮 (SLE)伴贫血的发生率、临床及实验室检查特征、误诊情况。方法 :对10 3例SLE患儿进行回顾性分析 ,将其中 78例伴有贫血的患儿分为两组 ,一组为自身免疫性溶血性贫血组 (AIHA组 ) ,另一组为非AIHA组 ,进行临床症状及实验室检查的比较。结果 :10 3例SLE中贫血发生率为 75 .7%(78/ 10 3) ;AIHA组和非AIHA组在发病年龄、性别上无显著性差异 ;在临床表现上 ,伴AIHA的SLE常以贫血为首发症状或主要表现 ;实验室检查上AIHA组血中IgG、补体C3 、球蛋白 (Glb)、总蛋白 (TP)降低比非AIHA明显。结论 :贫血是SLE常见的表现 ;SLE伴AIHA者 ,其免疫功能紊乱更明显 ,狼疮活动程度高 ;以AIHA的表现为首发症状或主要表现的SLE患者 ,易误诊。更多还原

【Abstract】 Objective:To investigate the incidence, clinical characteristics, laboratory indicators, and state of misdiagnosis among cases of SLE complicated with Anemia in children. Methods: A retrospective survey was made on 103 children with SLE, among which 78 cases complicated with anemia. These 78 cases were divided into 2 groups. One was autoimmune hemolytic anemia (AIHA) and the other non-AIHA. A research was made to compare the clinical characteristics and laboratory indicators of the tow groups. Results: Of 103 cases, 78 patients (75. 7%) were complicated with anemia. The AIHA and non-AIHA groups show no significant difference in sexuality and age. Anemia was usually the first or cardinal symptom of SLE complicated with AIHA. The levels of serum immunoglobin-G (IgG), complement 3(C 3), globulin and total protein were found to be significantly descent in the AIHA group. Conclusions: Anemia was the common symptom of SLE. The immunologic disturbance in SLE patients complicated with AIHA was much more serious than that in SLE patients without AIHA. A SLE patient with a manifestation of AIHA as the first or cardinal symptom was tended to be misdiagnosed.更多还原
