【摘要】 目的 :分析全髋关节置换术后髋关节疼痛与关节假体松动的关系。方法 :自 1991年 8月至 2 0 0 1年 4月 11年期间我院行全髋关节置换术 132例病人 ,共计 137髋。失访 31例 ,2例高龄病人死于与手术无关内科疾病。剩余 99例病人 (10 2髋 )年龄范围 :35岁～ 78岁 ,平均年龄 6 1 5岁。平均随访时间 6 9年。髋关节疼痛分为轻、中、重三度。将 99例病人 10 2髋分为无松动、X线松动、临床松动三组进行全髋置换术后髋关节疼痛的分析。结果 :X线松动组与无松动组之间髋关节疼痛发生率无显著差异 (P >0 0 5 ) ,临床松动组与无松动组及X线松动组之间髋关节疼痛发生率有显著差异 (P <0 0 5 )。且髋关节疼痛程度与假体松动程度成正相关。由于假体松动而导致的髋关节疼痛占疼痛髋关节总数的 5 8 6 %。结论 :假体松动是引起全髋关节置换术后髋关节疼痛的主要原因更多还原

【Abstract】 Objective To analyze the relationship between hip pain and prosthetic loosening after total hip replacements(THR).Method Between August 1991 and April 2001,137 total hip replacements were performed in 132 patients.Thirty one patients have been lost to follow up and two great aged patients died from unrelated diseases.In the rest,the average age of the patients at the time of the operation was 61 5 years (range,thirty five to seventy eight years).Follow up ranges from twelve months to eleven years with a mean of 6 9 years.The degree of the hip pain was graded into mild,moderate and severe.102 hips in 99 patients were divided into non loosening group,X Ray loosening group and clinical loosening group according to radiograph for analysis of the hip pain after THR.Results There is not significant difference in the incidence of the hip pain between X Ray loosening group and non loosening group( P >0 05),but there is significant difference between clinical loosening group and non loosening group,X Ray loosening group( P <0 05).Degree of the hip pain is in correlation with the degree of the prosthetic loosening.The ratio of the painful hips in the loose group to the total painful hips after THR is 58 6%.Conclusion The prosthetic loosening is the main cause of the hip pain after total hip replacements.更多还原
