基因重组葡激酶治疗幼猪急性脑梗死

【摘要】 目的 研究重组葡激酶 (rSaK)治疗急性脑梗塞的药效学。方法 幼猪 72头 ,分 7个静脉组 ,3个动脉组及 2个对照组。幼猪脑梗塞 4h后分别经腹壁静脉和颈部动脉导管应用不同剂量的rSaK ,用药结束后 0 5、 1 0、 1 5h测颈内动脉血流量并行脑血管造影。 2 4h后取脑作病理检查。结果 静脉组 0 8万U/kg或以上剂量的血管再通率均大于对照组 (P <0 0 5 ) ;相同剂量动脉组与静脉组血管再通率几乎相同。血管完全再通、部分再通及不通的脑梗塞发生率分别为 6 38%、 33 3%、 10 0 % (P <0 0 5 ) ,未见脑出血并发症。结论 静脉用rSaK治疗急性脑梗塞的最低有效剂量为 0 8万U/kg ,用药途径对疗效无明显影响

【Abstract】 Objective To study the thrombolytic effect of rSaK in a swine acute thromboembolic stroke model.Methods 72 piglets were divided into 7 intravenous thrombolysis groups(dosage from 4 000 to 64,000 U/kg),3 intraarterial thrombolysis groups(dosage groups:8 000,1 6000 and 3 2000 U/kg),2control group. 6 piglets were available in each group.rSaK in different dosages were given through abdominal venous and internal carotid artery(ICA) catheter after the distal part of ICA has been embolized for 4 hours.Blood flow of ICA was measured and DSA performed 0 5h,1 0h and 1 5h after.The brain was removed for patholoy after 24 hours.Results Recanaliyation rate of obstructed vessels for intravenous thrombolysis groups at 8 000 U/kg or more was higher than that for control group ( P <0 05);At the same dosage,reopening rate for intraarterial groups was almost the same as that for intravenous group.Incidence of cerebral infarct for fully Was 6 38%,for partially re canaliyed 33 3%,and unopened 100% if no recanaliyation ( P <0 05).Conclusion The lowest effective dosage of rSak as thrombolytic therapy in piglets thromboembolic stroke was 8 000 U/kg given intravenously.
