【摘要】 目的 阐述十二指肠旁疝的术前诊断及处理原则。方法 回顾性分析了急诊入院十二指肠旁疝7例。5例急诊剖腹探查,2例观察治疗6～8h后手术。术中证实为十二指肠旁疝,行疝环松解及缝合关闭术。2例行坏死肠段切除,4例行撕裂浆膜修补术。结果 6例恢复正常,2周后进软食出院。1例术后2周进普食出现轻度不全梗阻。结论 十二指肠旁疝临床少见,术前诊断困难。应仔细询问病史,密切观察,术中正确处理,提高诊治水平。更多还原

【Abstract】 Objective To elaborate the pre - operative diagnosis and principle of management of paraduodenal hernia. Methods A total of 7 cases of paraduodenal hernia were analysed retrospectively. All cases were admitted from emergency clinic and proved to be paraduodenal hernia after exploratory laparotomy. Among them 5 cases were undergone emergency exploratory laparotomy and 2 cases were conservatively treated for 6 - 8 h and then operated. Relaxation and suture the hernia analus were performed. Among them excision of the necrotic part of intestine was performed in 2 cases and repair the ruptured serosa in 4 cases. Results Six cases were recovered and discharged two weeks after operation and might take soft diet. One case when taking ordinary diet slight obstruction occurred. Conclusion Paraduodenal hernia is difficult to diagnose before operation, attentive history taking, pay close attention to the patient, proper management during operation are essential.更多还原
