【摘要】 目的:探讨夫妇间人类白细胞抗原—DR(HLA-DR)抗原相容性在习惯性流产中的意义。方法:采用PCR/SSP技术对24例原因不明性习惯性流产(UHA)患者及其配偶和28例正常对照者及其配偶进行HLA-DR分型分析。同时,采用丈夫静脉血的B淋巴细胞对UHA患者进行免疫治疗及观察。结果:24例UHA患者中有22例与配偶间HLA-DR型有1个以上的抗原相同;28例正常对照者中有5例与配偶间有1个以上的抗原相同;两组比较,差异有显著性(P<0.05)。UHA患者治疗前的抗父体的淋巴细胞毒性抗体(APCA)阳性率与对照组比较,差异有显著性(P<0.01);UHA患者治疗前后APCA阳性率比较,差异有显著性(P<0.05)。UHA患者免疫治疗后妊娠成功率为80％。免疫治疗后出生的新生儿无低体重儿及畸形儿。结论:HLA-DR抗原相容性在夫妇间增大,可能是UHA发病的重要原因。更多还原

【Abstract】 Objective: To investigate the mutual antigen of the human leukocyte antigen - DR (HLA - DR) a-mong the habitual abortion couples. Methods: HLA - DR in twentyfour couples with unexplained habitual a-bortion(UHA) and in twentyeight couples with ordinary pregnancy and delivery (the controlled group) were analysed by PCR/SSP(Polymerase chain reaction/Sequence specific permer). Twentyfour UHA women were immunized by their husband B - lymphocytes for 3 months. Results: Twentytwo couples among twentyfour couples with UHA had HLA - DR mutual antigen over one twain. Five couples of twentyeight couples with ordinary pregnancy and delivery had HLA - DR mutual antigen over one twain. The significant difference existed between two groups ( P < 0. 05). The UHA group’s anti - paternal lymphocytotoxic antitody ( APCA ) positive rate was 8% before immunization, which was significantly lower than that in the control group (86% ). After immunization, the APCA positive rate increased to 755 among the UHA couples. 20 cases achieved pregnancy and 16 deliverd normal newborns. Conclusion: The increase of the mutual antigen between couple may be the important cause of UHA.更多还原
