【摘要】 目的　研究高选择性周围神经 (包括坐骨神经和闭孔神经 )切断术治疗痉挛性脑瘫的临床应用。方法　应用高选性周围神经切断术结合传统的肌腱和骨性手术 ,对 2 5例有行走潜能的痉挛性脑瘫患儿进行治疗。并利用统一的评价标准对患儿术前术后的状态进行对比。结果　 2 5例术后患儿肌张力高症状得到很大程度缓解 ,剪式步态消失或缓解 ,尖足畸形得到矫治 ,下肢各肌群肌力趋于平衡 ,步态稳定性加强。 6周拆石膏时足部及关节外形好且稳定性强。 3月复查时各肌群力量恢复良好 ,锻练效果满意 ,姿势性痉挛大为改观。 6月时可扶物行走或独立行走。结论　高选性周围神经切断术治疗痉挛性脑瘫效果良好 ,可行性高 ,应加以推广更多还原

【Abstract】 Objective To study the clinical application of highly selective peripheral neurotomy in treating children with spastic cerebral palsy.Methods 25 CP patients who might walk without assistance were treated with the highly selective peripheral neurotomy and traditional operation to tendons and bones.Their pre operative and post operative status were valued by the same evaluation criterion.Results 15 post operative children were followed up.The symptom of high muscle tension was relieved after operation.Scissor gait was improved or disappear.Deformity of club foot was corrected.The muscular strength of lower limb became more equilibration and the stability of gait was strengthened.When we dismantled the plaster 6 weeks after the operation,we found that the shape of the feet and joints were good.3 months after the operation,the muscular strength of lower limb recovered well and the symptom of spasm was changed greatly.6 months after the operation,the children can walk themselves or with the help of others.Conclusion The highly selective peripheral neurotomy is effective to spastic cerebral palsy.更多还原
