乐昌市调整加碘盐浓度后重点人群碘营养水平调查分析
Survey on iodine status of key population after readjusted iodized salt concentration in lechang city
【摘要】 目的 了解和评价现行加碘盐浓度是否满足严重缺碘地区不同人群的碘营养需求。方法 选择严重缺碘地区的乐昌市市区(县城)和农村作为调查点,调查8-10岁学生、育龄妇女、孕妇、哺乳妇女及其婴幼儿的尿碘、盐碘、乳汁碘含量和甲状腺肿大率。盐碘采用滴定法测定,尿碘、乳汁碘采用砷铈催化分光光度法测定。结果 197份盐碘中位数为34.9mg,/kg,合格碘盐食用率为97.6%。8-10岁学生甲状腺触诊肿大率为3.4%。5种人群平均尿碘中位数为241.5μg/L,其中婴幼儿、育龄妇女、哺乳妇女、8-10岁学生、孕妇的尿碘中位数分别为286.6μg/L(n=198)、266.8μg/L(n=197)、260.5μg/L(n=200)、227.6μg/L(n=205)、177.7μg/L(n=200),尿碘高于300μg/L的比例分别为49.0%、44.7%、41.5%、29.3%、21.0%;仅8-10岁学生和哺乳妇女尿碘县城高于农村(P<O.05)。47份乳碘中位数为153.1μg/L。结论 现行的加碘盐浓度足以保证5种重点人群的碘营养需求,但婴幼儿、学生、育龄妇女和哺乳妇女的碘营养水平有偏高趋势,因此,建议将现行加碘盐浓度35±15mg/kg下调为30±10mg/kg。
【Abstract】 Objective To estimate whether the present iodine content in salt can meet the need of the iodine nutritional status in key population in severe iodine deficiency areas. Methods A town and a village were selected in severe iodine deficiency areas of Lechang city in Guangdong province. Key populations were schoolchildren aged 8-10 year, women at childbearing-age, pregnant women, breast-feeding women and their newborns. Iodine content in salt, urine and milk was tested and goiter prevalence was measured. Milk and urine samples were measured with arsenic-cerium catalyzed spectrophoto technique and iodized salt tested by means of titration neutralization. Results The median of 197 salt iodine samples was 34.9 mg/kg and the consumption rate of qualified iodized salt was 97.6% . The goiter prevalence in school children was 3.4% . The urinary iodine median in 5 key population was 241.5 μg/L and the medians in newborns, women at childbearing-age, breast feeding women, schoolchildren, pregnant women were 286.6μg/L( n = 198) , 266.8μg/L( n = 197) , 260.5 μg/L( n = 200) , 227.6 μg/L( n = 205) and 177.7μg/L( n = 200) respectively. The proportions of higher urinary iodine ( > 300 μg/L ) in these population were 49.0%、44.1% 、41.5%、29.3% and 21.0% respectively. Urinary iodine in pupils aged 8-10 year and breast feeding women in town were higher than that in village ( P < 0.05) . The median of 47 milk iodine samples was 153μg/L. Conclusion The iodine content in iodized salt at present can ensure the iodine nutrition in 5 key population. But there was a tendency of higher iodine status in newborns, schoolchildren, women at childbearing-age and breast-feeding women. We suggested to adjust the present salt iodine content from 35±15mg/kg to 30±10 mg/kg.
