肝癌经皮酒精注射术后肝再生的研究

【摘要】 目的:研究经皮酒精注射术(PEI)后,周围叶的正常肝脏组织是否存在肝细胞再生,及其再生的特点。方法:VX2肝癌动物模型兔50只,分别行经皮酒精注射术和肝癌肝叶切除术(PH),分别检测术后24h、72h的兔肝组织的有丝分裂指数(MI),以流式细胞术(FCM)行DNA含量分析,测定S期细胞比率(SPF),DNA指数(DI)的变化。结果:PEI术后24小时兔肝有丝分裂指数、S期细胞比率、DNA指数较对照组均明显升高(P<0.001);术后72h,有丝分裂指数、S期细胞比率较对照组均无明显差异(P>0.05),DNA指数较对照组升高(t=2.78,P>0.05)。并且术后24h,PEI术后肝细胞增殖明显弱于肝癌肝叶切除术,各项指标差异明显。结论:PEI术后兔肝组织存在一定程度的再生,弱于肝叶切除术后肝再生,其原因可能与酒精的局限性扩散和肝内酒精浓度升高作用有关。

【Abstract】 Objective:To study the perifocal liver regeneration and its characteristics after percutaneous ethanol injection of hepatic carcinoma.Methods:Fifty rabbits with VX2 liver tumor undergoing percutaeous ethanol injection and hepatic lobectomy,were measured 24h and 72h later the mitosis index(MI),S-phase cell fraction (SPF) and DNA index (DI) of the normal liver tissue by the flow cytometry (FCM).Results:MI,SPF and DI of the rabbit with VX2 liver tumor at 24h postoperatively,were significantly different from those of the control group(P<0.001),72h later MI and SPF of VX2 liver tumor were not significantly different from those of the control group(P>0.05),but DI of VX2 liver tumor was higher than that of the control group(t=2.78,P<0.05).Moveover,the proliferation of the liver cells of the rabbit treated by PEI was less than that by hepatic lobectomy,and all the indexes were significant different between those two treated groups.Conclusion:The liver regeneration of the rabbit with VX2 liver tumor after PEI is weaker than that by hepatic lobectomy,and after the cause may be associated with the local diffusion and high concentration of the ethanol in the liver.
