【摘要】 目的 :报告非开胸食管内翻拔脱后胃代食管术治疗下咽癌、颈段食管癌的术后并发症 ,探讨并发症的发生原因及预防措施。方法 :回顾性分析 1994年～ 2 0 0 0年间对 2 1例下咽癌、颈段食管癌病人行非开胸食管内翻拔脱后胃代食管术治疗情况。其中颈段食管癌 9例 ,全部保留喉功能 ;下咽癌 12例 ,其中 7例同时行喉全切除术 ,5例行喉部分切除术。全部病例均行幽门成形术。结果 :6例发生并发症 :吻合口瘘 1例 ,胃坏死 2例 ,肺炎 2例 ,心衰 1例。其中术后死亡 2例 ,并发症发生率 2 8.6 %。结论 :对并发症的发生原因、预防措施进行了讨论更多还原

【Abstract】 Objective: To clinically analyze the postoperative complications on replacement of esophagus with stomach after resection of hypopharyngeal and cervical esophageal carcinoma. Methods: Twenty one patients with hypopharyngeal and cervical esophageal carcinoma were treated by replacement of esophagus with stomach after the non\|open\|chest inverting avulsion of cervical esophagus in our department from 1994 to 2000. There were 15 males and 6 females, ranging in age from 41 to 73 years old. Among 21 patients, 12 were hypopharyngeal carcinoma and 9 were cervical esophageal carcinoma. The clinical data of 21 patients were analyzed retrospectively. The procedure of laryngeal function reserve was completed in 9 patients with cervical esophageal carcinoma. Among 12 patients with hypopharyngeal carcinomas, 7 patients were total\|laryngectomy and 5 were partial\|laryngectomy. The replacement of esophagus with stomach and pyloroplasty were completed in all patients. Results: The 2\|year survival rate was 72.7%(8/11) in hypopharyngeal carcinoma, 37.5%(3/8) in cervical esophageal carcinoma. The complications were found in 6 patients, which included stoma fistula in 1, stomach necrosis in 2, pneumonitis in 2 and heart failure in 1. The incidence of complications was 28.6% in this group. Conclusion: The replacement of esophagus with stomach after resection of hypopharyngeal and cervical esophageal carcinoma was a successful procedure for those patients, but the complications should be paid attention to avoiding.更多还原
