角膜中央厚度对高眼压症和开角型青光眼眼内压测量值的影响
Effect of central corneal thickness on the measurement of intraocular pressure in the patients with ocular hypertension and primary open angle glaucoma
【摘要】 研究角膜中央厚度对高眼压症和原发性开角型青光眼病人的眼内压测量值的影响。方法 :对 44例 (88眼 )临床诊断为高眼压症的患者 ,31例 (6 1眼 )原发性开角型青光眼患者及 5 2例 (10 4眼 )与高眼压症患者屈光状态相近的正常人的角膜中央厚度及眼内压进行检测 ,用统计学方法比较 3组的角膜中央厚度的差异及高眼压症患者的眼内压值与角膜中央厚度的相关关系。结果 :1高眼压症组的角膜中央厚度的平均值 5 85 .6± 35 .7μm明显高于原发性开角型青光眼 (5 4.1± 38.7μm) (P<0 .0 1)及正常对照组 (5 35 .5± 2 9.4μm) (P<0 .0 1) ,而原发性开角型青光眼的角膜中央厚度与正常对照组相比无显著性差异 (P<0 .0 5 )。 2将高眼压症患者的角膜中央厚度与眼内压测量值进行简单线性相关分析 ,可见角膜中央厚度与眼内压测量值呈明显正相关 ,相关系数为 0 .6 36 (P<0 .0 1)。角膜中央厚度每增加 5 0μm可使眼内压测量值升高约 2 mm Hg。结论 :1高眼压症患者的角膜中央厚度明显大于原发性开角型青光眼和正常眼 ,高眼压症患者的角膜中央厚度与眼内压呈明显的正相关 ,提示高眼压症患者是由于角膜中央厚度高于正常范围而造成眼内压测量值的升高。 2角膜中央厚度值可作为高眼压症及原发性开角型青光眼的诊断与鉴别诊断的重?
【Abstract】 Objective: To study the effect of central corneal thickness on the measurement of intraocular pressure in the patients with ocular hypertension (OHT) and primary open angle glaucoma (POAG) clinically and statistically Methods: Central corneal thickness (CCT) and intraocular pressure (IOP) were measured in 44 patients (88 eyes) with OHT, 31 patients (61 eyes) with POAG and 104 eyes of normal subjects as controls who had the relative similar refractions to those OHT and POAG patients. SPSS statistic software package was applied for the statistic analysis. Results: (1) CCT in patients with OHT were significantly greater than those in POAG or controls (P<0. 01). There was no significant in the CCT difference between POAG and normal control group (P>0. 05). (2) There was a positive correlation between CCT and IOP in the patients with OHT, the correlation coefficient was 0.636 (P<0. 01). Increasing 50μm of CCT would cause 2mmHg higher of IOP Conclusions: (1) The CCT of patients with OHT is significantly greater than those with POAG and normal controls. There is a significant correlation between CCT and IOP in OHT patients. (2) Routine CCT measurement should be considered as an important additional parameter for the clinical diagnosis and differential diagnosis for OHT and POAG. (3) When CCT is within the range of 520±50, the IOP measured seems relatively accurate. If CCT is over 570μm, IOP values might be over the normal limit and the increased CCT might cause the effect of OHT, which should be considered clinically
