丹参对感染性休克兔保护作用的实验研究

【摘要】 目的:观察丹参处理感染性休克模型后的循环代谢指标及模型肺标本光镜和电镜下的病理改变。方法:选择30只健康家兔,随机分为5组:假手术组,空白对照组,常规对照组,纳洛酮组,丹参治疗组。通过结扎盲肠末端并造成穿孔,复制与临床类似的家兔感染性休克模型后,给予相应的治疗,4h 后经颈总动脉置管测平均动脉压(MAP),并抽血测定乳酸盐、丙二醛及动脉血气;18h 后活杀动物取肺标本。结果:纳洛酮组、丹参治疗组可明显地升高 MAP,两组间差异无显著性;并可明显降低休克模型的血乳酸盐、及丙二醛的水平,耐受缺氧的能力明显强于常规对照组。丹参治疗组的肺水含量明显低于对照组。光镜见丹参治疗组的肺瘀血较轻,炎细胞浸润亦明显轻于常规对照组,电镜观察见该组的线粒体膜仍保持完好,未见破裂或空泡化,而常规对照组的线粒体膜则有明显的断裂、解体及线粒体的空泡化。结论:纳洛酮组、丹参治疗组均可明显改善感染性休克的代谢变化,但电镜显示肺泡上皮的超微结构变化表明丹参治疗组耐缺氧的能力明显强于常规对照组,可明显改善休克状态,保护肺脏功能。因此丹参治疗组在改善肺组织的病理形态学方面远较纳洛酮组为好,疗效更佳。

【Abstract】 To observe the pathological changes of lung sample in septic shock model treated by Salvia miltiorrhiza(SM)with the circulatory-metabolic parameters under light and dectron microscope.Methods:The 30 rabbits randomly chosen were divided into 5 groups:sham operation group,blank control group,routine control group,naloxone group and SM treated group.By means of ligating the end of caecum to cause perforation simulating septic shock model in rabbits.Four hours later,measuring mean aortic pressure (MAP)via carotid common artery intubation,and the level of lactate,malonylaldehyde(MDA)and blood gas were determined,18 hrs later,sacrificing the rabbit to obtain the lung sample.Results:MAP can be raised markedly in the naloxane group and the SM treated group,there was no difference between these 2 groups.The levels of blood lactate and MDA which is the product of arachidonic acid metabolism were reduced.The anoxia resistant capability was obviously stronger than that of the control,while the lung water content of the treated group was markedly lower than that of the control.The lung blood stagnancy and inflammatory cells infiltration were milder than the control groups under light microscopy;the mitochondrial membrane of treated group kept intact without rupture or vac- uolation as the changes of control.Conclusion:Naloxane control group and SM treated group could improve the metabolic status of sep- tic shock models,but their ultrastructural change under electron microscopy revealed that in the treated group,resistance against anoxia was much better than that in the control.SM treated group could improve shock status and protect organ’s functions.It was much better than naloxane control group in reversion of the pathomorphology of lung tissue,the therapeutic effect of the former was also better.
