【摘要】 目的 :探讨经导管封堵PDA后残余分流对溶血的影响。材料及方法 :分析 6例PDA封堵术后机械性溶血者 (溶血组 )即时残余分流、肺动脉显影程度及浓度、堵塞器位置。随机抽取 10例封堵术后无溶血者对照分析 (对照组 )。结果 :溶血组术后主动脉造影示主肺动脉全程及肺动脉窦显影 ,中、远 1/3段显影浓度与降主动脉相当。对照组肺动脉窦均无显影 ,3例主肺动脉无显影 ;4例远 1/3段浅淡显影 ;3例远 1/3段显影浓、近、中 1/3段显影淡。结论 :残余分流量大是术后机械性溶血的重要原因 ,正确选择堵塞器大小及准确置入是最佳预防方法更多还原

【Abstract】 Purpose: To evaluate the effect of hemolysis and residual shunt in patients with PDA treated by transcatheter closure. Material and Medthods: Hemolysis and residual shunt in 6 patients with PDA treated by transcatheter closure (A group) were reviewed. The enhancement of the extent and the density of the pulmonary artery and the location of the duct occluder devices were analyzed on aortogram following the operation of closure of PDA with occluder immediately. 10 patitnts without hemolysis following transcatheter closure of PDA were selected randomly as control group (B group). Results: In A group, whole main pulmonary artery and pulmonary artery sinus were enhanced on aortogram. The density of the middle to the distal 1/3 section was enhanced as same as that of the descending aorta. In B group, the enhancement of pulmonary artery sinus was not found. Pulmonary artery was not enhanced in 3 cases. The distal 1/3 section of main pulmonary artery was enhanced mildly in 4 cases, and the distal 1/3 section of pulmonary artery evidently and the middle 1/3 section mildly in 3 cases. Conclusion: Vast residual shunt is important value for patients with hemolysis at post transcatheter closure of PDA. Hemolysis could be prevented in those patitnts with the duct occluder device in the correct size and the correct location.更多还原
