【摘要】 目的 :探讨HANS辅助硬膜外麻醉用于妇科全子宫切除手术中的临床效果。方法 :60例病人随机分为两组 ,均使用常规的两点法硬膜外麻醉 (T1 2 ～L1 向上和L3～ 4向下置管 ) ,一组使用HANS ,另一组则不用作为对照。主要观察指标包括辅助药的使用率 ,镇静评分 ,牵拉反射 ,血压 ,心率和氧饱和度等。结果 :两组病人麻醉手术中使用同样的处方 ,药物用量近似 ,术中辅助药物使用 ,病人镇静评分 ,牵拉反射 ,血压 ,心率的变化有较明显的差异 ,对照组 93.3%病人均要使用辅助药物 ,由于辅助药物的使用 ,术中镇静评分较高 ,总平均达 6.7分 ,特别在用药后病人呼之不醒 ,1 /2的病人出现明显的舌根后坠 ;HANS组使用率仅为 1 0 % ,术中各项生理指标相对平稳 ,两组氧饱和度差别不明显。结论 :HANS辅助硬膜外麻醉用于全子宫切除术可显著减少牵拉反应和辅助药的使用 ,术中各项生理指标较平稳更多还原

【Abstract】 Objective: To investigate the clinical effect of HANS on epidural anesthesia of two points for uteroectomy. Method: 60 cases,aged from 35 yrs to 48 yrs, were divided randomly into two groups,30 cases each. Group C received epidural anesthesia only, Group H epidural anesthesia with HANS (DD wave,continuous during anesthesia and operation). Observatory index included BP and HR emotional scale,SaO 2,pelvic reflection and sedation. Results: There was no diffrence in anesthesia block segment, volume of bolus and operative times between the two groups, but there were significant differences in sedation administration (10%∶93.3%) and sedation scale(2.3∶6.7) due to much more sedatives used in group C. The fluctuation of BP and HR was significantly higher in Group C, and the incidence of stretch reflex (86.7%) was much higher in group C than in group H (10%). Conclusion: Patients were much comfortble during the operation under epidural anesthesia with HANS and vital signs were more stable, sedative administration was much less in the group using HANS.更多还原
