【摘要】 目的：比较异丙酚、硫喷妥钠、依托咪酯静脉诱导、气管插管期间对循环及内分泌的影响。方法：全麻病人30例，随机分为三组( n =10)，分别给予异丙酚(Ⅰ组)、硫喷妥钠(Ⅱ组)、依托咪酯(Ⅲ组)行静脉诱导、插管，观察其平均动脉压、心率、血糖、皮质醇、胰岛素在诱导、插管期间的变化。结果：心率： II组、III组在插管即刻明显增快，(Ｐ<0.05)。平均动脉压： Ⅰ组在插管前下降明显，(Ｐ<0.05)。 Ⅱ组在插管前下降和插管后4min明显上升，(Ｐ<0.05)。Ⅲ组在插管即刻有明显上升，(Ｐ<0.05)。皮质醇、胰岛素： Ⅱ组、Ⅲ组在插管后4min有明显上升，(Ｐ<0.05)。血糖：3组在诱导插管期间均无明显变化，(Ｐ>0.05)。结论：异丙酚静脉诱导比硫喷妥钠、依托咪酯对循环及内分泌的影响小，应激反应轻，静脉诱导平稳，适用于各种全麻手术的诱导。更多还原

【Abstract】 Objective: To compare the influence of haemodynamic and hormonic responses to induction and endotracheal intubation with propofol, thiopental and etomidate. Methods: 30 patients were divided randomly into three groups ( n =10). Anesthesia was induced with either propofol 2mg/kg ( groupⅠ), thiopental 5mg/kg ( groupⅡ) or etomidate 0.3mg/kg ( groupⅢ), together with fentanyl 3μg/kg and vecuronium 0.1mg/kg for endotracheal intubation. Results: The HR of group I was decreased (Ｐ>0.05) after induction , the groups Ⅱ and III heart rate were increased significantly during the intubation, (Ｐ<0.05 ). The MAP of group Ⅰ was decreased after induction, ( Ｐ<0.05 ). The group Ⅱ MAP had decreased after induction and increased during and after 4min intubation, ( Ｐ<0.05 ). The group Ⅲ MAP was increased during intubation, ( Ｐ<0.05 ). In groups Ⅱ and Ⅲ, both cortisol and insulin were increased obviously after intubation for 4 min, (Ｐ<0.05) In all the three groups, the blood sugar was not significantly increased after intubation, ( Ｐ>0.05). Conclusion: Haemodynamic and hormonic responses are less in propofol group than in thiopental and etomidate group during induction and intubation and it is useful for anesthesia to do the induction with propofol.更多还原
