土默特地区三个自然村居民饮水砷暴露与恶性肿瘤
A Study on Death of Malignant Tumor and Arsenic Exposure to Drinking Water in Three Village of Tumote Area
【摘要】 目的对内蒙古呼和浩特土默特地区三个自然村居民恶性肿瘤死亡状况进行回顾性分析 ,了解恶性肿瘤死亡率与饮水砷浓度的关系。方法采用回顾性队列研究方法 ,自 1975年 1月 1日至 1997年 12月 31日历时 2 3年 ,按接触水砷浓度将 2 140名调查对象分为 :<0 .0 5 m g/ L 组、0 .0 5~ 0 .5 0 mg/ L 组和≥ 0 .5 0 mg/ L 组。结果饮水砷浓度≥ 0 .5 0 mg/ L 组的恶性肿瘤死亡位于第一位 ,死亡率为 983.84/ 10万人年 ,显著高于 0 .0 5~ 0 .5 0 m g/ L 组(6 1.2 5 / 10万人年 )和 <0 .0 5 mg/ L 组 (6 4.2 3/ 10万人年 ) ,其相对危险度是 0 .0 5~ 0 .5 0 mg/ L 组的 12 .92倍 ,是 <0 .0 5 mg/ L 组的 12 .17倍 ,归因危险百分比分别为 92 .2 6 %、91.78%。≥ 0 .5 0 m g/ L 组恶性肿瘤死亡顺位为肺癌(5 2 9.76 / 10万人年 )、肝癌 (15 1.36 / 10万人年 )和食管癌 (75 .6 8/ 10万人年 )。同时发现水砷摄入总量与恶性肿瘤死亡间有相关关系。结论饮水砷暴露与恶性肿瘤死亡率之间存在着较强的联系。说明砷的远期危害是不容忽视的
【Abstract】 Objective In order to study the relationship between the m ortality of malignant tumor and arsenic content of drin- king water in three villa ges of Tumote area of Huhhot,Inner Mongolia.Methods This retrospecti ve cohort study analyzed the data during 23 years period from Jan 1,1975 to Dec 31,1997.According to arsenic concentration in their drinking water,2 140 cases w ere divided into <0.05 mg/L group,0.05~0.50 mg/L group and ≥0.50 mg/L group.Results The malignancy death in arsenic exposure ≥0.50 mg/L group(983. 84 per 100 thousand person years) ranked the first and was obviously higher than that in arsenic exposure 0.05~0.50 mg/L group(61.25 per 100 thousand person yea rs) and <0.05 mg/L group(64.23 per 100 thousand person years).The relative risk of malignancy mortality in arsenic exposure ≥0.05 mg/L group was 12.92 times a s high as the arsenic exposure 0.05~0.50 mg/L group and 12.17 as <0.05 mg/L grou p.The attributable risk percent was 92.26% and 91.78%,respectively.The mortality of lung cancer(529.75 per 100 thousand person years) in arsenic exposure ≥0.50 mg/L group ranked the first of the death of malignancy and liver cancer(151.36 per 100 thousand person years) ranked the second and esophagus cancer(75.68 per 100 thousand person years) the third.A correlative relationship was found betwee n the total content of arsenic ingested and malignancy death.Conclusion A significant relationship was observed between arsenic exposure of drinking water and malignancy mortality.It suggests that it should not be allowed to ign ore the arsenic long-term harm.\;
