脑卒中偏瘫患者急性期的综合康复治疗

【摘要】 目的探讨综合康复治疗对脑卒中急性期患者肢体运动功能恢复和 ADL 能力改善的影响。方法选择发病3 d内入住神经内科病房的脑卒中偏瘫患者 88例 ,分为康复组 48例 ,对照组 40例 ,2组一般治疗相同 ,康复组同时配合促进技术、心理治疗和集体康复训练等综合康复疗法 ,于治疗前后对 2组患者分别进行 Brunnstrom分级和Barthel指数评定运动功能和 ADL 能力。结果综合康复疗法能明显改善患者的运动功能和 ADL 能力 ,2组比较差异有显著性 (P<0 .0 5 )。结论以促进技术为主的综合康复措施有助于脑卒中偏瘫患者尽快恢复功能 ;个体化的康复方案安全可靠 ,经济实用 ,适用于一般医院神经科病房患者急性期的康复 ,更适用于基层医院 ,值得向家庭和社区推广应用

【Abstract】 Objective To study the effects of comprehensive rehabilitation on the recovery of extremities motor function and the ADL ability of the stroke patients with hemiplegia at the acute stage. Methods Eighty eight patients who had come down for less than 3 days were randomly assigned to rehabilitation group ( n =48) and control group ( n =40). All of them received routine treatment. Further more, the patients in the rehabilitation group were treated with facilitation techniques, kinesitherapy, psychotherapy and collective training,. Brunnstrom scale and Barthel index were used to assess motor functions and ADL abilities beforee and after training. Results Extremity motor function and ability of ADL were obviously improved after comprehensive rehabilitation. There was significant difference between the two groups ( P <0 05). Conclusion The effects of comprehensive treatments, especially facilitation techniques, were evident in the patients with stroke. The individualized rehabilitation is safe, economical and practical. It’s applicable to grass roots hospitals, and worth being populatized to family and community.
