胫骨骨折不同手术固定器材的生物力学研究及其临床意义

【摘要】 目的 :确定不同固定器材在胫骨骨折固定中的优缺点。方法 :将 30具胫骨标本制成中段横行无缺损骨折模型 ,行单侧外固定支架 (UADF)、Crosse -Kempf钉、TCP三种器材固定后 ,测试各项生物力学性能。结果 :抗压刚度GK钉最强 ,弯曲刚度TCP最强 ,UADF此二项刚度差 ,抗扭刚度三者无差异。结论 :UADF生物力学性能差 ,TCP易产生应力遮挡 ,GK钉固定坚强 ,术后能早期下地 ,弹性模量适中 ,应力遮挡小 ,较符合生物力学固定原则。

【Abstract】 Objective: To clear the virtues and defects of different fixaion instruments in fracture of tibia.Methods:Thirty tibial specimens were made indefectible in middle part.The fracture samples were fixed differently by unilateral axial dynamine fixation (UADF),Grosse-Kempf nail and trapezoid compression plate (TCP).Each performance of biomechanics was done.Result:The degree of anti-compression shows that Grosse-Kempf nail is the firmest,and antibending power shows that TCP is the firmest.In these two powers UADF is the weakest.There is no significant difference in torsional rigidity power in these three groups.Conclusion:According to the biomechanics function,UADF is poor,and TCP can easily lead to stress shelter.While Grosse-Kempf nail can achieve the goal of firm fixation and its anti-bending power is moderate,but its stress shelter is the least.Grosse-Kempf nail can accord with the demand of biomechanics.
