细粒棘球蚴原头节18ku纯化抗原ELISA的建立及其对两型包虫病鉴别诊断的意义
ESTABLISHMENT OF ELISA TEST WITH 18 ku PURIFIED ANTIGEN FROM ECHINOCOCCUS GRANULOSUS PROTOSCOLECES AND ITS DIAGNOSTIC VALUE FOR THE DIFFERENTIATION OF THE TWO TYPES OF ECHINOCOCCOSIS
【摘要】 目的 用细粒棘球蚴原头节 18ku纯化抗原建立 EL ISA诊断方法 ,用于两型包虫病的鉴别诊断。 方法 用18ku纯化抗原 EL ISA方法对 44例泡型包虫病 (AE)、70例囊型包虫病 (CE)、2 9例囊虫病和 30例健康人血清中特异性抗体进行检测 ,同时用羊包囊液抗原 (Bu)常规 EL ISA法检测上述血清做比较。 结果 18ku纯化抗原检测不同血清的阳性率为 :AE90 .91%、CE11.43%、囊虫病和健康人血清均为阴性 ;Bu抗原分别为 :AE97.73%、CE88.5 7%、囊虫病5 1.72 %和健康人 10 .0 %。 18ku- EL ISA对 AE血清诊断敏感性为 90 .91%、特异性 93.80 %、阳性预示值为 83.33%、阴性预示值 96 .80 %。 结论 两型包虫病患者体内 18ku抗体水平存在明显差异 ,纯化抗原 18ku- EL ISA可用于两型包虫病的鉴别诊断 ,较免疫印渍法简便快速。
【Abstract】 Objective To establish the ELISA test to diagnose the two types of echinococcosis using 18 ku purified antigen from Echinococcus granulosus protoscoleces, and a comparative study has been done by using ELISA and westernblot. Methods A total of 173 sera from 44 patients with alveolar echinococcosis (AE), 70 cystic echinococcosis (CE), 29 cysticercosis and 30 healthy controls was tested by ELISA with 18 ku antigen and crude cystic fluid antigen collected from CE infected sheep. Results The positive rate in 18 ku-ELISA was 90.91% for AE cases, 11.43% for CE, and none of positive for cysticercosis and healthy controls, while the positive rate in crude cystic fluid antigen was 97.73% for AE cases, 88.57% for CE, 51.72% for cysticercosis and 10.0% for healthy controls, respectively. The overall diagnistic values of 18 ku-ELISA for AE sera were sensitivity 90.91%, speciticity 93.80%, positive predictive value 83.33% and negative predictive value 96.80%. Conclusion 18 ku-ELISA test could be used for differentiation of AE from CE patients based on their significant differences of antibody levels with relatively high sensitivity and specificity. In practicable, 18 ku-ELISA test is more convenient than 18 ku-Immunoblot analysis.
