气管放疗加深低温保存后同种异体移植的实验研究

【摘要】 目的 探讨气管大剂量放疗加深低温保存后 ,同种异体节段性气管移植的可行性。方法 健康杂种犬 2 2只分两组。对照组 (10只 )供体气管不做任何处理。实验组 (12只 )供体气管放疗 10万cGy后深低温 (- 196℃ )保存 ,同种异体气管移植长度为 6个气管软骨环 (4cm) ,移植后两组动物均不使用免疫抑制剂。结果 对照组动物均于移植后 2~ 3周因气管狭窄窒息死亡或处死 ,肉眼标本见移植段气管明显狭窄、挛缩、部分软骨脱落。放疗加深低温保存组动物于移植后 1个月内无狭窄症状 ,部分动物于术后 2个月出现不同程度的狭窄 ,气管内置管后临床症状缓解 ,半数实验组动物移植段气管无狭窄发生 ,内膜光滑 ,吻合端愈合良好 ,动物长期生存。结论 气管大剂量放疗加深低温保存方法可降低同种异体移植的免疫原性 ,经大剂量放疗加深低温保存的气管移植后可不使用免疫抑制剂 ,是一种有临床应用前景的方法。

【Abstract】 Objective: To investigate the possibility of tracheal allotransplantation using high dose irradiated and cryopreserved grafts in canine model. Methods: Twenty -two adult mongrel dogs were divided into 2 groups. In group I (n=10) the grafts were not treated. In group II (n=12), the grafts were irradiated with 100?000 cGy and cryopreserved (-196℃). A 5~6 ring (4cm) section of thoracic trachea was allografted without immunosuppression. Results: All animals of group I either died of asphyxia or were killed due to severe stenosis of graft 2~3 weeks after allotransplantation. Postmortem examination of these grafts showed stenosis, shrinkage and absorption of tracheal cartilage. In group II no animals showed tracheal stenosis within 1 month. 4 animals showed various degrees of stenosis after 1 5~2 0 months. Histological examination showed, the grafts were covered with normal epithelium. Six animal suvived long term with normal tracheal structure and viability. Conclusion: Irradiation and crypreservation of allograft could reduce antigenicity. Tracheal allotransplantation with high dose irradiated and cryopreserved graft without use of Immunosuppression is promising for clinical use.
