异常妊娠羊水中促红细胞生成素的测定及其与脐血中促红细胞生成素相关性研究

【摘要】 目的 检测羊水中促红细胞生成素 (EPO)的浓度及脐血中 EPO浓度。探讨异常妊娠羊水中 EPO浓度与胎儿宫内缺氧的关系。 方法 应用化学发光酶标免疫测定法对 30例妊娠高血压综合征 (简称妊高征 )患者 ,30例妊娠合并糖尿病患者及 2 5例正常妊娠者之剖宫产儿的羊水 EPO浓度及脐静脉血清 EPO浓度进行定量检测。同时检测脐动脉血 (Ua) p H、碱剩余 (U a BE)及 Uap O2 等指标。 结果 妊高征组 ,糖尿病组及对照组羊水 EPO及脐血 EPO浓度实测值的自然对数转换值分别为1.42± 0 .33与 1.6 3± 0 .37;1.5 3± 0 .2 5与 1.81± 0 .34;0 .91± 0 .11与 1.13± 0 .14,前两组均高于对照组 ,差异有非常显著性 (P<0 .0 1)。三组 Uap H、U a BE及 U ap O2 分别为 7.2 49± 0 .0 49、(- 5 .1± 2 .1)mm ol/ L 与 (2 .0 0± 0 .2 8) k Pa(1k Pa=7.5 mm Hg) ;7.2 40± 0 .0 6 8、(- 5 .3± 2 .2 ) m mol/ L 与 (2 .0 4±0 .33) k Pa;7.2 72± 0 .0 0 7、(- 3.0± 1.3) mmol/ L 与 (2 .36± 0 .41) k Pa,前两组均低于对照组 ,差异显著 (P<0 .0 5 ,P<0 .0 1)。各组羊水 EPO浓度与同组脐血 EPO浓度呈显著正相关 (r值分别为 0 .84、0 .79及 0 .6 4,P<0 .0 1)。异常妊娠组 L n UVSEPO与 Uap H及 U a BE均呈显著的负相关 (r值分

【Abstract】 Objective To measure the concentration of the amniotic fluid EPO(AFEPO) and investigate the relationship between AFEPO levels and fetal hypoxia. Methods Their AFEPO and the umbilical vein serum EPO (UVSEPO) were measured thirty pregnancy induced hypertension pregnancies (PIH group) , 30 gestational diabetic pregnancies (Diabetic group) and 25 normal pregnancies (control group) were terminated by cesarean section by using a chemiluminescent enzyme labeled immunometric assay (CELIMA). Meantime, umbilical artery pH (UapH), base excess(UaBE) and UapO2 and so on were also measured. Results The mean concentrations of AFEPO and UVSEPO in PIH (1.42±0.33 and 1.63±0.37) and Diabetic group (1.53±0.25 and 1.81± 0.34) were significantly higher than those in control group (0.91±0.11 and 1.13±0.14), ( P < 0.01). The mean values of UapH , UaBE and UapO2 in PIH 7.249±0.049, (-5.1±2.1)mmol/L and (2.00±0.28)kPa, respectively and Diabetic group 7.240±0.068, (-5.3±2.2)mmol/L and (2.04±0.33)kPa, respectively were significantly lower than those in control group [7.272±0.007, (-3.0±1.3)mmol/L and (2.36±0.41)kPa, respectively], ( P<0.05,P<0.01 ). Furthermore, the levels of LnAFEPO and LnUVSEPO correlated highly significantly in all three individual groups ( P <0.01) . LnUVSEPO correlated negatively and significantly with UapH and UaBE in PIH ( r= -0.46, P<0.05 ; r=-0.64 , P<0.01 ) and Diabetic group( r=-0.68,P<0.01; r= -0.74, P<0.01 ). Conclusions The umbilical vein serum and so amniotic fluid EPO levels were highly correlated. AFEPO reflects closely of fetal UVSEPO. So AFEPO can be used as an antepartum indicator of chronic fetal hypoxia.
