【摘要】 目的研究腰椎间盘再手术前影像学特点对术式选择的指导作用。方法对57例腰椎间盘再手术前影像学资料作系统分析 ,明确腰椎管骨性增生程度 ,残留关节突完整性、下腰椎稳定性、神经根管大小 ,有无神经根病变如肿瘤、畸形 ,瘢痕压迫的严重程度等 ,据此选择相应手术方式 ,并与术中病理发现对照 ,随访观察术后疗效。结果52例术前影像学诊断与手术探查结果相同 ,影像学诊断与术后诊断符合率91 %。手术疗效优良率为84 %(48/57)。结论腰椎间盘再手术前影像学特点的系统分析对术式选择具有不可缺少的指导作用更多还原

【Abstract】 Objective To study the correlation between prereoperiative imaging manifestations of lumbar disc herniation and operative patterns.Methods 57 cases were studied, according to imaging manifestations, analysing the protrusive position and type of lumbar disc herniation,the bony hyperplasia of vertebral canal, the integrity of remaining articular processes, the stability of low lumber vertebral, the size of the canal of nerve roots, the lesion of nerve roots , the extent of scar compression, ie. The operative patterns were chosen mainly according to the prereoperative imaging manifestations of lumbar vertebral.Results The imaging diagnoses of 52 cases(91%)are proved identical to the pathological changes by operation. 84%(48/57)are excellent or good.Conclusion Before taking appropriate operative patterns,it is essential to make a systemical analysis of the imaging manifestations.更多还原
