脑动静脉畸形的3D-CTA形态学评价
Preliminary evaluating the value of three dimensional computed tomographic angiography in diagnosis of the morphology of intracranial arteriovenous malformations
【摘要】 目的 评价 3D CTA对脑动静脉畸形 (AVM )的影像学诊断价值。方法 2 3例脑AVM病例同时行 3D CTA与DSA检查 ,两者对照研究。结果 2 3例脑AVM术前 3D CTA检查阳性诊断率 10 0 % ,无假阳性和假阴性。与DSA比较 :(1)供血动脉的来源、数目和形态判断完全吻合 ;(2 )引流静脉判断完全吻合 19例 (83 % ) ,部分吻合者 4例 (17% ) ;(3)病灶部位判断完全吻合。无并发症发生。结论 脑AVM的 3D CTA二维影像与DSA基本吻合 ,立体空间三维形态描述好 ,可弥补后者不足 ,对手术有指导价值。本技术无创、低风险、价廉 ,其技术进一步成熟后可取代DSA
【Abstract】 Objective To assess the value of three dimensional computed tomographic angiography (3D CTA) which used in the diagnosis of the morphotogy of intracranial arteriovenous malformations(AVMs). Methods A prospective evaluation within 2 years included 23 patients suspected of intracranial AVMs. All the patients underwent 3D CTA and digital subtraction angiography (DSA). Results The 3D CTA imaging provided excellent visualization of the intracranial AVMs. The false positive error and false negative error were not found. The details of arterial supply, size and location, which provided by 3D CTA images were exactly as the same as that in DSA, and the details of venous drainage were approxiamte match. Moreover, 3D CTA can provide a 3 D anatomy of AVMs and the relationship with adjacent structures, which can assist the surgeon to possible intropperative design. As a non invasive technique, there was no related complication . Conclusions 3D CTA provide similar 2 D images compare to images of DSA. Furthermore, it also supplies several advantages over DSA, in addition to reduced costs and avoidance of related complications. Preliminary data suggest that 3D CTA can partial replace catheter angiography inthe assessment of patients with intracranial AVMs. Although no firm conclusions or generalizations can be drawn from this limit cohort of patients, it is hoped that this report will stimulate interest and further study in both regions of technique and clinic.
