两种低能量起搏电极的动态观察
Dynamic Observation of Two Low Energy Consumption Pacing Leads.
【摘要】 动态观察分形镀复电极 (FCL)和激素释放电极 (SEL)急、慢性期起搏阈值和阻抗的变化规律 ,为临床选择起搏电极和开始低能量起搏的时间提供参考依据。对 33例起搏器患者的 49条电极 ,其中FCL 2 5条、SEL 2 4条 ,用起搏系统分析仪和体外程控仪测定术中及术后 1,3,5 ,7,10 ,15 ,2 2天和 1,2 ,3个月的电压阈值和阻抗 ,计算各次的能量阈值。结果显示术中FCL的起搏阈值低于SEL(P <0 .0 5 )。术后FCL的起搏阈值逐日升高 ,第 10天达峰值 ,然后逐渐降低 ,2 2天时与SEL的起搏阈值相近。术后 2个月 ,FCL起搏阈值又开始低于SEL ,但无显著差异。SEL的起搏阈值始终保持在一个低水平 ,整个随访期无明显波动。两种电极均为低能量起搏电极 :SEL起搏阈值低而稳定 ,FCL的更低但峰值明显 ,两者可分别于术后拆线时和术后 3周开始低能量起搏
【Abstract】 To dynamicaly observe the pacing threshold and impedance between fractally coated leads (FCL) and steroid eluting leads (SEL)in the acute and chronic period postimplantation.The pacing parameters of 33 patients,25 FCL and 24 SEL were measured during the implantation with pacing system analysis (PSA) and on the lst,3rd,5th,7th,10th,15th,22th day and 1st,2nd,3rd month postimplantation with telemetry programmer.Intraoperative pacing threshold of FCL was lower than that of SEL( P <0.05).But the value of FCL increased gradually after implantation and arrived at the peak value on the 10th day,then it started to decrease.On the 22th day,the pacing threshold of these two leads were similar.On 2nd month postimplantation,pacing threshlod of FCL was lower than that of SEL again(NS).During the whole follow up peroid,the pacing threshold of SEL kept stable at the low level and had no significant variance.Both FCL and SEL are low energy consumption pacing leads.Pacing threshold of the former is lower than that of the latter but has a peak value.Low energy pacing should be started after 3 weeks postimplantation for the former and after the stitch is taken out for the latter.
