【摘要】 目的　评估吻合器环形痔切除术治疗脱垂性内痔的安全性和实用性。方法　利用特制的环形吻合器对 5 2例Ⅲ、Ⅳ度环形脱垂性内痔进行手术治疗 ,其中男性 2 4例 ,女性 2 8例 ,平均 6 2岁。结果　手术时间平均8min ,术后住院时间平均 3 5天 ,98%的病人对手术效果满意。吻合口距齿线距离愈近术后痔块回缩愈明显 ,但术中吻合口出血的情况愈多 ,术后肛门部不适的主诉愈多。术后常见的并发症 :尿潴留 34例 (6 5 4% ) ,肛门部疼痛 42例 (80 7% ) ,大便带血 2 0例 (38 5 % ) ,下腹不适及胀痛 12例 (2 3 1% ) ,术后 1周左右有排便困难 8例(15 4% ) ,大便次数增多 4例 (7 7% ) ,肛门部水肿 4例。随访 2周至 10个月 ,1例术后 4个月复发 ,无肛门狭窄、大便失禁发生。结论　与传统手术相比吻合器环形痔切除术治疗脱垂性内痔具有手术简单、术后痛苦少、恢复正常生活早、复发率低、并发症少的特点 ,但远期效果尚待进一步观察更多还原

【Abstract】 Objective To evaluate the effectiveness of the new technique using circular stapler in the treatment of hemorrhoidal disease.Methods A transversal mucohemorrhoidal transection was performed using a circular stapler above the anorectal joint,enabling radical surgery to be completed in a single stage with a rapid and mini-invasive technique.52 patients with third and fourth degree hemorrhoidal prolapse were enrolled in the study,aged 62 years old in average.Results The mean operating time was 8 minutes and hospital stay was 3 5 days after operation.98% of the patients were satisfied with the results.The closer the distance of the anatamosis to the dentate line, the better effect of the correction of the prolapsed hemorrhoid,but the more bleeding intra-operatively and more complaints post-operatively.The common complications were urine retention(65 4%),mild anal pain (80 7%),mild anal bleeding (38 5%)and lower abdominal discomfort(23 1%).Conclusions Hemorrhoidectomy for prolapsed hemorrhoid with stapler is a simple,safe procedure with good results,low complications and early recovery,but the long-term effect is to be investigated.更多还原
