一氧化氮、内皮素及其比值与糖尿病肾病的相关研究

【摘要】 目的 探讨 2型糖尿病 (DM)患者一氧化氮 (NO)、内皮素 (ET)及其比值的变化规律和在糖尿病肾病 (DN)发病中的作用。方法 测定 76例 2型 DM患者与 40例正常人血中 NO、ET水平 ,计算 NO/ ET比值 ;采用 Student- t检验 ,单因素直线相关分析及多因素分析。结果  1NO/ ET值在正常白蛋白尿组 (NA)显著升高 (P<0 .0 1) ,微量白蛋白尿组 (m A)和大量白蛋白尿组 (MA)明显降低 (P<0 .0 5 )。 2 NO/ ET与病程、UAER(自然对数值 )呈负相关 (P<0 .0 1) ;ET与病程、U AER呈正相关 (P<0 .0 1)。 3NO/ ET、NO及体重指数 (BMI)、吸烟、舒张压 (DBP)、文化程度为 DN发病的主要危险因素。结论  2型糖尿病患者血 NO/ ET比值是 DN发生发展的主要危险因素之一。

【Abstract】 Objective The effects of the NO, ET and NO/ET value on the pathogenesis and progress of diabetic nephropathy (DN) were investigated in the study.Methods The circulating levels of NO and ET were measured in 40 normal subjects and 76 cases with type 2 diabetes mellitus (DM). The student t test,single variate and multivariate analyses were implemented to study the relationship between the regulative factors and DN.Results ①NO/ET value was higher in normal albuminuria group (NA) ( P <0.01), and lower in microalbuminuria (mA) and macroalbuminuria group (MA) than that in normal control (NC) ( P <0.05). ②Values of NO/ET and NO were inversely related to the duration of DM and UAER( P <0.01). Plasma ET level was positively related to duration of DM and UAER( P <0.01). ③Multivariate analysis by Cox model showed that the values of NO/ET and NO were the important risk factors in the pathogenesis and progress of DN.Conclusion The NO/ET value was one of the important risk factors in pathogenesis and progress of DN.
