胎儿肾积水12例超声研究

【摘要】 目的 :研究胎儿肾积水的不同特点及转归 ,防范不必要的中止妊娠。方法 :利用 B超对 12例 18只集合系统分离径 >1.0 cm积水胎肾进行随访观察和研究。结果 : 组 :为可复性肾积水 ,10例 16只 ,积水宽度在 1.0 1~1.6 3cm之间 ,肾实质较厚在 1.0 2~ 0 .5 8cm之间 ,此类胎儿肾积水于分娩后即日或一到二月后相继消失 ; 组 :为不可复性肾积水 ,2例 2只 ,集合系统分离径为 2 .15 cm和 2 .5 6 cm,实质较薄为 0 .3cm和 0 .2 cm,随访至出生后 1.5年和2 .0年仍为重度积水表现。结论 :晚妊胎儿肾积水宽度 <1.6 3cm或实质厚度 >0 .5 8cm者应视为可复性 ;积水宽度 >2 .15 cm或肾实质厚度 <0 .2 cm者为不可复性的肾积水 ,出生后应及时治疗。

【Abstract】 Objective:To study the different characteristics and changes of fetus hydronephrosis(FH),and prevent unnecessary induced abortion.Methods:12 cases 18 individuals hydronephrotic kidneys whose collecting system(CS)are seperating were observed and studied.Results:Group Ⅰ:These hydronephrotic kidneys could return to normal,and these were 10 cases 16 individuals.Whose widths of hydronephrosis were from 1 01 to 1 63 cm,and the thickness of renal parenchyma were from 1 02 to 0 58 cm.These hydronephrosis had disappeared in one to two months after delivered.Group Ⅱ:These hydronephrotic kidneys could not reuturn to normal,and these were 2 cases 2 individuals.whose widths of hydronephrosis were from 2 15 to 2 56 cm.and the thickness of renal parenchyma were from 0 3 to 0 2cm,these hydronephrosis had existed heavily in one and half years to two years after delivered.Conclusions:If the width of CS of FH was narrower than 1 63cm,or the thickness of renal parenchyma was wider than 0 58cm.It should be regarded as it could be returned to normal.But if the width of hydronephrosis was wider than 2 15cm,or the thickness of renal parenchyma was narrower than 0 2cm.It should be regarded as it could not be returned to normal,and after birth should be treated in time.
