【摘要】 目的 :通过应用 B超测量精囊腺大小及精液检测 ,探讨单纯性射精管梗阻性无精子症的非创伤性诊断。方法 :经腹部 B超测量此类患者的精囊腺大小、并检测精液量、精液 p H值和精浆果糖浓度 ,与正常有生育能力的男性进行比较。结果 :患者的精囊腺分别为 :右侧上下径 (2 3.1± 5 .7) mm ,前后径 (9.1± 1.7) mm,左侧上下径 (2 5 .1± 5 .4) mm,前后径 (10 .3± 2 .4) mm。果糖浓度 (2 3.2 2± 2 0 .6 4) mg/ dl、精液 p H值 6 .48± 0 .18、精液量 (0 .72± 0 .47) ml。正常男子的精囊腺为 :右侧上下径 (33.0± 5 .1) mm,前后径 (14.0± 2 .8) mm,左侧上下径(33.5± 5 .2 ) mm,前后径 (13.3± 2 .0 ) mm。果糖浓度 (2 74.2± 12 6 .7) mg/ dl、精液 p H值 7.2± 0 .19、精液量(2 .95± 2 .6 4) ml。结论 :单纯性射精管梗阻性无精子症患者的精囊腺比正常男子小 ,两者之间差异有显著意义 (P<0 .0 1) ;梗阻性无精子症患者 ,若精囊腺明显偏小 ,且精液 p H值 <7.0 ,则可以推断为单纯性射精管梗阻性无精子症更多还原

【Abstract】 Objective:To explore the diagnosis of nontraumatic azoospermia due to simple ejaculatory duct obstruction.Methods:The size of seminal vesicle in the azoospermic patients was measured by transabdominal B ultrasound,and the semen volume,pH value and seminal fructose level were also tested.The value of normal fertiles was also measured as controls.Results:The average sizes of patients seminal vesicles were:right longitudinal diameter 23 1±5 7mm,anteroposterior diameter 9 1±1 7mm;left longitudinal diameter 25 1±5 4mm,anteroposterior diameter 10 3±2 4mm;and in normal fertiles,right longitudinal diameter 33 0±5 1mm,anteroposterior diameter 14 0±2 8mm,left logitudinal diameter 33 5±5 2mm,anteroposterior diameter 13 3±2 0mm.The values of seminal fructose were 23 22±20 64mg/dl and 274 2±126 7mg/dl,pH values were 6 48±0 18 and 7 2±0 19,semen volumes were 0 72±0 47ml and 2 95±23 64ml respectively.There were significant differences between the two groups.Conclusions:The size of seminal vesicle in the azoospermic patients due to simple ejaculatory duct obstruction is smaller than that in normal fertiles( P <0 01).If the semen volume in the obstructive azoospermic patients is less than 2ml,and pH<7 0 simultaneously,the diagnosis of azoospermia due to simple ejaculatory duct obstruction may be suggested.更多还原
