【摘要】 目的 :测定正常人与门静脉高压症 (PH)患者腹腔动脉、肝动脉、脾动脉血流参数 ,观察 PH断流术前后各血管血流动力学变化。方法 :应用彩色多普勒测量 115例正常人、 46例 PH患者及 43例 PH患者断流术前后的腹腔动脉、肝动脉、脾动脉的血流参数 ,并对其进行统计学处理。结果 :PH患者与正常人比较 ,肝动脉血液流速变慢 (P<0 .0 5 )、血流量减少 (P<0 .0 1) ,脾动脉直径增粗 (P<0 .0 1)、血流量增多 (P<0 .0 1)。 PH患者断流术后肝动脉血流量增多 (P<0 .0 5 ) ,腹腔动脉流量减少 (P<0 .0 5 )。术后门静脉血栓 (PVT)发生率为 2 7.9% (12 /43) ,5 0 %发生于左支。结论 :PH时肝动脉血流量 (HAF)减少 ,脾动脉血流量明显增加。断流术后肝动脉血流量较术前增加更多还原

【Abstract】 Objective:To measure the hemodynamics of the celiac artery,hepatic artery and splenic artery in normal subjects and patients with Portal hypertension(PH),and also the hemodynamics of celiac artery and its branches in PH pre-and post-portaazgous disconnection(PD).Methods:We measured the diameters and hemodynamics of celiac artery,hepatic artery and splenic artery in 115 normal subjects,46 patients with PH and 43 patients with PH before and after PD with color Doppler sonography.Results:Compared with those of the normal subjects,the hepatic arterial flow(HAF)of PH was decreased and slowed(P<0.01).The diameter of splenic artery became widened and splenic arterial flow was increased(P<0.01).The hepatic arterial flow(HAF)was increased after PD(P<0.05),and the celiac arterial flow was decreased (P<0.05).The incidence of the patients with portal venous thrombosis(PVT)were 27.9%(12/43).Conclusions:Hepatic arterial flow(HAF)was decreased in PH and HAF was increased after PD.更多还原
