【摘要】 目的　探讨免疫印迹法幽门螺杆菌分型诊断与不同上消化道疾病以及抗幽门螺杆菌治疗疗效的关系。方法　应用免疫印迹和14 C呼气试验观察了来我院就诊的Ⅰ型和Ⅱ型幽门螺杆菌现症感染者中胃炎、消化性溃疡和胃癌构成比以及三联抗幽门螺杆菌治疗根除率。结果　Ⅰ型幽门螺杆菌现症感染者中消化性溃疡和胃癌的构成比显著高于Ⅱ型 (P <0 0 0 1)。三联治疗对Ⅰ型与Ⅱ型幽门螺杆菌的根除率相似 (分别为 81 5 %和 85 7% ,P >0 0 5 )。结论　应用免疫印迹法进行幽门螺杆菌分型诊断有利于临床对上消化道病情的判断 ,Ⅰ型感染可能更有治疗的必要性 ,三联治疗对Ⅰ型和Ⅱ型幽门螺杆菌均有较好的效果更多还原

【Abstract】 Aim To investigate the relationship of different serotypes of Helicobacter pylori with gastritis,peptic ulcer and gastric cancer and with the effects on eradication treatment.Methods 14 C breath test was used to diagnose the present infection of Helicobacter pylori(Hp) 200 patients with present Hp infection were serotyped as type Ⅰ and type Ⅱ by immunoblot technique.The presence of any antibodies to 128KD?116KD?110KD?95KD?91KD?87KD Hp proteins was defined as type Ⅰ The percentage of gastritis,peptic ulcers and gastric cancer in patients with Hp type Ⅰ or type Ⅱ was investigated respectively.Results In distribution,the percentage of peptic ulcers and gastric cancer in patients with type Ⅰ Hp infection was significantly higher than that in patients with type Ⅱ Hp infection (47 7% and 16 3% respectively, P <0 001) In treatment,totally,triple treatment had similar good eradication rates for both type Ⅰ and type Ⅱ Hp infections (81 5% and 85 7% respectively, P >0 05) Conclusions Immunoblot serotyping of Hp may be a helpful index for judging the severity of the gastric and duodenal disorders caused by Hp infections.It seems more necessary to eradicate the type Ⅰ Hp infection,and both types of Hp infection are sensitive to triple treatment.更多还原
